34 -- SUPPLY STRUCTURAL PLATE BOX CULVERT FOR THE PAYETTE NATIONAL FOREST SOL Commercial Item: RFQ 12QR4-099-99 DUE 071599 POC Alice A. Muffley, Contracting Officer, 208-373-4133 This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quotation number 12QR4-099-99. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-11. Set aside for small businesses -- SIC 3443, Size Standard: 500. Line item as follows: Supply a structural plate box culvert having the approx. dimensions of 40'-5" span, a rise greater than 11' and less than 13'-6" and a length of 50'. Product standard is Super Cor Box Culverts as provided by Atlantic Industries Limited, or approved equal. The manufacture must furnish design drawings and calculations for HS-25 load rating in addition to a minimum 2' and a maximum of 5' of cover with a unit weight of 135 lbs/cf above rise dimensions as defined by Super Cor definition. Certification of HS-25 loading will appear on the drawings. Unbalanced channel footing supports and installation instructions must be supplied in advance. All items provided must comply with the Forest Service Specifications for Construction of Roads and Bridges (Aug 1996). The box culvert must be delivered to the Fourmile Crossing site on the Payette National Forest, 28 miles east and 17 miles north of Cascade, Idaho via Warm Lake Highway and FDR 474/674 respectively. Delivery will be 45 calendar days after award. A bonus of 10% of the total contract amount will be given if delivery is made by August 15, 1999. The Contractor will make a knowledgeable member of their staff available on site for 2 days during assembly. A 10-day notice will be given as to the dates required and 2 days during backfill. This requirement will be completed within 1 year of the delivery of the box culvert.
