Q -- DENTAL HYGEINIST SOL RFQ 124-0021 DUE 080699 POC Richard Houser, Contracting Officer 608-584-6120 The Federal Correctional Institution, Oxford, Wisconsin is soliciting to establish a contract with a Dental Hygienist to provide services for the inmate population at said facility. Services to include but not limited to; providing oral hygiene education, dental prophylaxis, evaluation and assessment of dental pathology, exposing and developing of roetgenograms, removal of soft and hard deposits, scaling, root planning, curettage and assisting the dentist in select periodontal procedures. Contractor to provide two hour sessions, eight sessions per week, for an estimated 418 sessions for the expected contract period of October 1, 1999 through September 30, 2000. Offerors must be a graduate from an approved school of dental hygiene with current state of Wisconsin licensure and/or certification to be considered for award. The offeror shall furnish copies of all state licenses, board certifications, continuing education received during the contract period, proof of professional liability insurance, and diploma(s) pursuant to JCAHO standards and failure to comply with this requirement will result in the offer being found "non-responsive" and award of contract will not be made. A firm-fixed price contract will be awarded on/or about September 7, 1999 to the lowest qualified bidder. This acquisition is 100% set-aside for small business.
