65 -- LITHOCLAST PNEUMATIC LITHOTRIPTER SYSTEM SOL 553-46-99 DUE 090399 POC Diane Foster (313) 576-4281 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the subparts in Federal Acquisition Regulation 12.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation number Request For Quotes (RFQ) 553-46-99 is assigned and shall be referenced on any quotes. This solicitation document and incorporated provisions and cluases are those in effect through Federal Acquisition Circular 97-13. The Standard Industrial Classification is 3841. Contractor shall provide to the Department of Veterans Affairs, John D. Dingell Medical Center a pneumatic lithotripter system that uses pulsed waves to fragment all types of ureteral, bladder and kidney calculi including hard calcium oxalate monohydrate and cystine stones. Contractor shall provide this system to include control unit; power supply cable; quick connector air tube; quick connector without air tube; pneumatic foot control with tubing (2mm); handpiece (2); package of seals for handpiece; replacement silicone tube; silicone tube with quick connector(2mm); sterilization cap. Contractor to provide stainless steel probes (2.4fr) and (3.0fr). Contractor shall provide a master handpiece which includes silicone tube with connector; two sealing caps; two silicone probe guides; two standard probe caps; and storage case. Proposals shall be submittedc on an "all or none basis". Partial proposals shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately.
