SUBJECT: ADDRESSING ASTHMA IN HIGH-RISK COMMUNITIES Prospect Associates, Ltd., prime contractor for the NHLBI-sponsored Health Information Network seeks subcontractors (through local asthma coalitions) to conduct projects that will address performance goals of the National Asthma Education and Prevention Program (NAEPP). The goals are as follows: 1) promote asthma awareness, 2) diagnose and manage asthma, 3) implement community asthma programs and policies, and 4) prevent recurrent exacerbations of asthma. Offerors will be requested to submit proposals which seek to conduct performance projects in high-risk communities within Health Service Areas as defined by high asthma death rates. Performance projects may focus on community outreach activities as well as implementation of aspects of the NAEPP's clinical practice guidelines. It is anticipated that three to five subcontracts will be awarded for a period of 3 years each. This is NOT a Request for Proposals (RFP). RFPs will be solicited by Prospect Associates, Ltd.
