This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplementad with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number N00189-99-R-0065 is a Request for Proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 12. The line items are as follows: Contract Period: 1 Oct 99 through 30 Sep 2000 Item Description Qty Unit 1001 On call maintenance service for FORE 12 MO $____ $_____ System ATM equipment and software support to include: (Gold Service) technical assist. 24 hours/7days per week, toll free call (fax and e-mail), support software upgrades, access to customer information service FORE worldwide web registered user service advance replacement of the hardware. 2002 1 Oct 2000 through 30 Sep 2001 3003 1 Oct 2001 through 30 Sep 2002 4004 1 Oct 2002 through 30 Sep 2003 The contractor shall propose for one base year plus three option years. The same description will apply for all years. The contractor shall request a copy of the following: a. Statement of Work b. List of the equipment to be manufactured c. FAR 52.212-3 (if you don't have a copy of the clause) All equipment shall be Y2 compliant. The contractor shall submit form DD254. All supplies shall be FOB destination to Norfolk, VA. Provision at 52.212-1, Instructions to Offerors-Commercial and any addenda applies. FAR 52.212-2, Evaluation-Commercial Item applies. The following factor shall be used to evaluate offers: (i) Technical capability of the items offered to meet the Government's requirement; (ii) Price; (iii) Past performance. Technical and past performance when combined are approximately equal to cost or price. The contractor shall submit a completed copy of 52.212-3, Offeror Representations and Certifications-Commercial Item, with its offer.
