janet.hudson@med.va.gov. This is a combined synopsis/solicitation for
commercial services prepared in accordance with the format in Subpart
12.6 as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation is being issued as Request for Proposal #565-3-00. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-12. The SIC code is
8011 and the small business size standard is $5.0 million. The VA
Medical Center, Fayetteville, N.C. intends to solicit proposals and
award a firm fixed price contract under the authority of Title 38 USC
Section 8153, pending availability of funds for services of a board
certified physiatrist in the Physical Medicine and Rehabilitation
Service of the VAMC, Fayetteville, N.C. Services will be provided on
site four (4) hours each week. The contract is for one (1) year with
two(2) one (1)year option periods to extend. The following services
will be provided in accordance with the terms and conditions of the
solicitation: furnish the services of a board-certified physiatrist on
site in the Physical Medicine and Rehabilitation Service four (4)
hours per week. Schedule of hours that services will be provided will
be mutually agreed upon between the contractor and the Chief of Staff's
Office. Physiatrist will act solely as a medical consultant to the
physicians at the VAMC. The physiatrist will not provide any type of
primary care services including intake examinations. Physiatrist will
not be on call as backup or primary care provider for any patients on
the VAMC's medical wards or clinics, nor be responsible for nor respond
to medical emergencies. The Physiatrist, not providing any primary
health care services but acting only as a consultant, will only advise
and recommend treatment and diagnostic testing. Inpatient
consultations for new patients will include a comprehensive history,
examination and treatment plan for therapy services. Physiatrist will
provide a review and recommendations for staffing of therapy
departments, not to include individual personnel review. Clinical
oversight in the PM&R Service will be provided. Contractor will act in
compliance with all Rules and Regulations of the Fayetteville VA
Medical Center. The contract physician will satisfy Veterans Health
Administration and medical center policies and JCAHO. The physician
must meet the requirements for appointment to the medical staff, VAMC,
Fayetteville, N.C. as outlines in the Medical Staff Bylaws; have a
current unrestricted license to practice in one of the states or
territories of the United States or District of Columbia. Personnel
performing these services shall be board certified and must meet the
credentialing privileging requirements of the VA Medical Center,
Fayetteville, N.C. Services to be performed by the contractor will be
under the direction of the Chief of Staff or his/her designee at the VA
Medical Center, Fayetteville, N.C. The following information is
provided as general information. The contractor under the terms of the
contract shall furnish the work hours as mutually agreed upon in the
work statement. Work hours: Monday through Friday -- 8:00 a.m. -- 4:30
p.m. National Holiday: The ten holidays observed by the Federal
Government, i.e. New Year's Day, Martin Luther King's Birthday,
Washington's Birthday, Memorial Day, Independence Day, Labor Day,
Columbus Day, Veterans Day, Thanksgiving, and Christmas Day and any
other day specifically declared by the President of the United States
to be a national holiday. Nonworking hours (normal on-call hours)
Monday through Friday, 4:30 p.m. to 8:00 a.m., and all Saturdays and
Sundays. Emergency On-Call Hours: 12:00 Midnight Sunday through 12:00
Midnight the following Sunday. No on-call, weekend coverage or after
hour services are to be performed unless specifically stated in the
awarded contract. Requests for on-call weekend coverage or after hour
services not stated in the contract must be approved by the Contracting
Officer and a contract modification issued prior to performing work.
Any work performed with prior approval of the Contracting Officer is
not reimbursable. The contractor shall be responsible for protecting
the personnel furnishing services under this contract. To carry out
this responsibility, the contractor shall provide the following for
their personnel: worker's compensation, professional liability
insurance, health examinations, and income tax withholding and social
security payments. The parties agree that such personnel shall not be
considered a VA employee for any purpose and shall be considered
employees of the contractor. A record keeping system will be developed
by the Chief of Staff's office or his designee of contractor hours
worked. The employee must sign in before commencing work and sign out
upon completion of work daily so that work can be certified for
payment. Payment for any leave, including sick leave, holiday or
vacation time is the responsibility of the contractor. The physician's
timecard will be authorized on a weekly basis by the Chief of Staff or
designee. A copy of the physician's timecard will be maintained by the
COTR for certification of payment upon receipt of invoice. Contractor
will be paid only for hours actually worked on site at the VAMC,
travel time is not authorized. The following provisions apply to this

announcement shall submit a cost proposal to include all inclusive cost
per hour x estimated 208 hours = cost for base year, option year 1 and
2. Technical proposal to include the following evaluation factors
company experience, personnel qualifications to include evidence of
current licensure and credentials/privileges and current CV, past
performance of contracts of similar type, scope size and complexity
that are ongoing or completed within past three years, references with
points of contacts for contracts identified. Technical and past
performance, when combined, are approximately equal to cost or price.
Award will be made to the offeror who offer conforming to the
solicitation will be most advantageous to the Government, price and
other factors considered. The information required in provisions
