is provided for a Personal services. This combined
synopsis/solicitation for commercial items is prepared in accordance
with FAR Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; quoatations are being requested and a written
solicitation will not be issued. This solicitation, number
N00259-99-Q-0158, is issued as a Request for Quotations (RFQ). The
solicitation document and incorporated provisions and clauses are those
in effect through FAC 97-12. This solicitation is 100% set aside for
small business. Services to be performed: On-site full-time two (2)
Cytotechnologists for the period of 01Oct99 through 30Sep00 in
accordance with the Statement of Work as shown in this notice. This
acquisition incorporates the following FAR clauses: 52.212-1
INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (OCT 95) incorporated by
reference with the following addenda: in paragraph (a) the government
will evaluateproposals based on the cost and technical capability.
Proposals must include a Certificate and two references within the last
year. 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERCIAL
ITEMS (OCT 95) must be completed and submitted along with the proposal,
a copy can be obtained by contacting the Purchasing Agent, Domingo B.
Custodio, Jr. at (619 532-8106. 52.232-36 PAYMENT BY THIRD PARTY (MAY
99). 52.232-18 AVAILABILITY OF FUNDS (APR 84). In accordance with FAR
clause 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT
STATUTES OR EXECUTIVE ORDERS-COMMERCIAL ITEMS (OCT 95). DFARS
252.204-7004 REQUIRED CENTRAL CONTRACTOR REGISTRATION (MAR 98). Offers
are due not later than 1200 hours of 09-20-99. LABOR CATEGORY.
ATTACHMENT ATTCHMENT I CYTOTECHNOLOGIST SERVICES 1.1. The contractor
shall provider services for: CYTOTECHNOLOGIST SERVICES PLACE FOR
PERFORMANCE: 2.1. The health care worker shall provide services for the
Naval Medical Center San Diego in the Laboratory. 3. DUTY HOURS 3.1.
The health care worker shall be on duty in the Laboratory Department
located at Naval Medical Center San Diego, for 40 hours per week. The
contractor shall normally provide services for an 8.5 hour period (to
include an uncompensated .5 hours for lunch) between the hours of 0615
and 1645. Specific hours will be scheduled by the Commander. -The
health care worker shall arrive for each scheduled shift in a well
rested condition and-shall have had at-least six hours of rest from all
other duties as a cytotechnologist in any setting immediately prior to
reporting for the shift. (Moonlighting will be expressly forbidden,
except with the prior approval of the proper chain of command in
directing the Chairman, Laboratory Department.at Naval Medical Center
San Diego). 3.2. When required, to ensure completion of services that
extend beyond the normal close of business, the health care worker will
remain on duty in excess of the scheduled shift. The health care worker
will be given an equal amount of compensatory time to be scheduled upon
mutual agreement of the health care worker and the Commander. 3.3.
Services of the health care worker shall not be required on the
following paid holidays: New Year's Day, Martin Luther King's Birthday,
Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus
Day, Veteran's Day, Thanksgiving Day, and Christmas Day. The health
care worker shall be compensated by the contractor for these periods of
authorized planned absence. 3.4. The Government shall not compensate
the contractor for any overtime services provided. 4. ABSENCES AND
LEAVE 4.1. Planned absences from assigned duties shall be requested
with 15 working days advance notice, in writing, to the Commander. Six
hours of annual leave are accrued by each health care worker at the
end of every 80 hour period worked. Leave accrues only to the
individual providing services. The health care worker shall be
compensated by the contractor for these periods of authorized planned
absence. 4.2. Unplanned absences due to illness or other incapacitation
of the health care worker will be allowed up to a maximum of 6.5 days
per contract year. Two hours of sick leave are accrued by each health
care worker at the end of every 80 hour period worked. Leave accrues
only to the individual providing services. The health care worker shall
follow the policy of the Commander regarding request and the use of
sick leave. If the health care worker is absent for three or more
consecutive days due to illness, he or she may be required by the
Commander to provide written documentation from a qualified health care
provider that he or she is free from communicable disease and the cause
of the health care worker's current illness. The Government reserves
the right to examine and/or re-examine any health care worker who meets
this criterion. The health care worker shall be compensated by the
contractor for these periods of authorized absence. 4.3. Any and all
sick and annual leave accrued by the health care worker will be
forfeited at the expiration of his or her period of performance. If the
contract is terminated for default, there will be no reimbursement for
any accrued leave balance. In the event that the health care worker
gives notice of employment termination, all accrued annual leave must
be used within that notice period, or forfeited. Sick leave taken
during this period shall be supported by a physician's statement of
illness upon request. 4.4. Only the Commander has the authorityto grant
leave to the health care worker that has not been.accrued,-in
accordance with the medical-treatment facility's (MTF) policies and
standards. -- 4.5.' A.uthoriza`tion for planned absences.may:be
gianted-by the Commander for attendance ~at continuing education
courses. The health care-worker shall be compensated by the contractor
for these periods of authorized planned absence. T~e Government will
not reimburse the health care 3;orkerfor the cost of any course tuition
and/o'r~other related education expense. 4.6. Documented military leave
for military reservists;will be allowed, not to exceed 15 calendar days
per calendar year, and may be taken intermittently, e.g., one day at a
time. Military leave will not be compensated leave. The health care
worker shall follow the policy of the MTF commander with respect to
notification of scheduled military duties to the Commander. 5.
INDIVIDUAL PROFESSIONAL FILE (IFP) 5.1. Upon award, the health
care-workers shall complete an Individual Professional File (IPF) prior
to performance of services. Completed IPF must be forwarded 30 days
prior to performance of duties to the MTF's Professional Affairs
Department. The IPF which will be maintained at MTF, contains specific
information with regard to qualifying degrees and licenses, past
professional experience and performance, education and training, health
status and competency as identified in Appendix (s) of BUMEDINST
6320.66B of Nov 97 and subsequent. revisions, and higher directives. A
copy of this instruction may be obtained by contacting Professional
Affairs at (619) 532-6460.. Primary source verification is the
responsibility of the contractor. 6. .GENERAL DUTIES AND
RESPONSIBILITITES. 6.1. Administrative and Training Requirements. The
health care worker shall: 6.1.1. Perform limited administrative duties
which include maintaining statistical records of workload,
participating in laboratory education programs, preparing documentation
according to workload reporting procedures, overseeing ordering of
supplies, ensuring efficient inventory control, and participating in
clinical staff quality assurance functions as prescribed by the
Commanding Officer. 6.1.2. Provide training and /or direction to
supporting government employees assigned to the health care worker
during the performance of clinical procedures. Such direction and
interaction will adhere to government and professional clinical
standards and accepted clinical protocol. Participate in clinical staff
quality assurance functions at the prerogative of the Commanding
Officer. The health care worker may be required to maintain statistical
records of the health care worker's clinical workload. 6.1.3.
Participate in monthly meetings to review and evaluate the care
provided to patients, identify opportunities to improve the care
delivered, and recommend corrective action when problems exist. Should
a meeting occur outside of the health care workersregular working
hours, the health care worker shall'be required to read and initia
l:the minutes of the meeting. 6.1.4. Participate in monthly inservice
training to MTF staff members. 6.1.5. Attend annual renewal of the
following Annual Training Requirements provided by the MTF: family
advocacy, disaster training, infection control, Sexual Harassment,
Bloodborne Pathogens and: Fire Safety. 6.1.6. FAMILY ADVOCACY.
Participate in the,implementation of the Hospital's Family
AdvocacyProgram as directed. 6.2. Clinical Duties. Routine workload is
scheduled by the Head, ;Laboratory Medicine Department. Primary
workload is related to the provision of medical care by staff health
care providers through writtenor verbal orders. 6.2.1. The health care
worker shall perform a full range of cytopathology technician
services, within the scope of this statement of work, on site using
government furnished supplies, facilities and equipment within the
assigned unit of the hospital. Workload occurs as a result of scheduled
and unscheduled requirements for services. 6.2.2. Clinical Functions.
The Cytopathology Branch provides full service cytopathology laboratory
services providing the screening and reporting of gynecologic smears,
nongynecologic specimens, and fine needle aspirations. The health care
worker's clinical activity will be a function of the overall demand
for cytopathology technician services. The health care worker's
productivity is expected to be comparable to that of other technicians
assigned the same facility and authorized the same scope of practice.
The health care worker shall: 6.2.2.1. Verify that smears, fluids and
fine needle aspirations submitted for cytological examination are
labeled according to the NMC Standard Operating Procedures (SOPs).
6.2.2.2. Accession specimens into the CHCS system. 6.2.2.3. Stain
slides appropriately with pap stain or Diff-Quick, apply coverslip and
label slides. Ensure that slides are matched up with appropriate
paperwork prior to submission to cytotechnologist for screening.
6.2.2.4. Use careful cytological technique to prepare high quality
cytospin preparations on various body fluid specimens. 6.2.2.5. Prepare
cytological specimens for cell blocks. 6.2.2.6. Prepare PLL-coated
slides for use with medical and FNA specimens. 6.2.2.7. Prepare and
document performance of daily stain line quality control and
maintenance. 6.2.2.8. Assist in ordering supplies for the
cytopreparatory laboratory and assure that the work area is
well-stocked and orderly. 6.2.2.9. Use principles of cytological
fixation and staining, knowledge of medical terminology and laboratory
information systems in performance of services under this contract.
6.2.2.10. Assist in ordering supplies for the cytopreparatory
laboratory and assure that the work area is well stocked and orderly.
6.2.2.11. Screen GYN and/or NonGYN slides at approximately 60 slides a
day in accordance with governing regulations (CCA, CLIP, and LAP)
6.2.3. The health care worker may direct supporting government
employees assigned to the health care worker during the performance of
applicable cytopathology services. The health care worker shall
provide training to the Naval Cytopathology School and the Naval School
of Health Sciences. This training shall include orientation,
instruction and supervision of cytotechnology students in correct
cytopathology procedures and technique. Additionally, the health care
worker shall instruct rotating cytotechnologists in the methods of
cytopreparation. 7. SPECIFIC MINIMUM PERSONNEL QUALIFICATIONS FOR
CTTOTECHNOLOGISTS QUALIFICATIONS: 7.1. Must be certified as a
cytotechnologist by the American Society of Clinical Pathologists and
hold state licensure as a cytotechnologist. 7.2. Experience ofat least
2 years experience within the preceding 3 years as a cytotechnologist.
7.3. A completed and updated Personal and Professional Information
Sheet (PPIS). 7.4 Provide three letters of recommendations from three
practicing physicians or laboratory supervisors attesting to your
skills. Reference letters must include name, title, phone number, date
of reference, address and signature of the individual providing
reference. Letters of reference must have been written within the
preceding 5 years. 7.5 Represent an acceptable malpractice risk to the
