synopsis/ solicitation for commercial items prepared in accordance with
FAR Part 13 and the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the ONLY solicitation; quotes are being requested and A
SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number/
RFQ N00244-99-T-B476 applies. This solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-12. The Government requires nine (9) Dental
Assistants and one (1) Dental Hygienist as follows: LineItem 0001:

Dental Assistants to perform Dental Assistant duties at the Naval
Dental Center or one of the branch dental clinics located at Camp
Pendleton, CA, from 21 September 1999 through 21 December 1999, in
accordance with the Statement of Work; Quantity Ordered: 4032; Unit:

time (40 hours per week, excluding Fed. Holidays) Dental Assistant to
perform Dental Assistant duties at the Naval Dental Clinic, MCAS
Miramar, CA, from 21 September 1999 to 21 December 1999, in accordance
with the Statement of Work; Quantity Ordered: 504; Unit: HRS; Unit

hours per week, excluding Federal Holidays) Dental Hygienist to perform
Dental Hygienist duties at the Naval Dental Clinic, MCAS Miramar, CA,
from 21 September 1999 to 21 December 1999, in accordance with the
Statement of Work; Quantity Ordered: 504; Unit: HRS; Unit Price:______;
Amount:______. STATEMENT OF WORK: A. GENERAL REQUIREMENTS: 1. The use
of "Commanding Officer" means: Commanding Officer, NDC Camp Pendleton,
CA, Commanding Officer, NDC Miramar, CA, or designated representative,
e.g. Technical Liaison, Department Head. 2. The services provided under
this contract shall be rendered as personal services for the Navy,
performing an agency function by providing direct medical care required
by the Navy for its health care beneficiaries. Prior to performance of
services, the contractor shall submit to the technical liaison
credentialing documents as required. The Contractor's activities shall
be subject to day-to-day supervision and control by Navy personnel in
a manner comparable to the supervision and control exercised over Navy
uniformed and civil service personnel engaged in comparable work. The
term "supervision and control" is defined as that process by which you
receive technical guidance, direction and approval with regard to an
element of work or a seriesof tasks within the requirements of this
contract. It is the intent of the parties that this personal service
contract create an employer-employee relationship between you and the
Navy. Accordingly, personal injury claims alleging negligence by the
contractor within the scope of the contractor's performance of this
contract shall be processed as claims alleging negligence by DOD
military or civil service employees. The contractor is not required to
maintain medical liability insurance. 3. Duties and Responsibilities.
The contractor shall perform a full range of dental hygiene and dental
assistant duties, within the scope of this statement of work, on site
using government furnished supplies, facilities and equipment within
the assigned unit of the Dental Treatment Facility. Workload occurs as
a result of scheduled and unscheduled requirements for care. The
contractor's actual clinical performance will be a function of the
Commanding Officer's credentialing process and the overall demand for
dental hygiene and dental assistant services. 4. The contractor shall
be subject to guidelines set forth in the Command's quality assurance
and risk management instructions. The contractor shall perform
administrative duties which include maintaining statistical records of
your clinical workload, participating in dental education programs,
preparing documentation for boards, and participating in clinical staff
quality assurance functions at the prerogative of the Commanding
Officer. 5. The work environment involves risks typically associated
with the performance of clinical oral procedures. The contractor may be
exposed to contagious disease, infections and flying dental debris
requiring the wearing of protection such as sterile gloves, masks and
eyeglasses. B. DENTAL ASSISTANTS LOCATED AT CAMP PENDLETON AND MIRAMAR:
1. The Dental Assistants shall be responsible for a full range of
dental assisting procedures in support of Dental Officer examinations
and delivery of treatment under the "four handed dentistry" format
within the personnel and equipment capabilities and limitations of the
Dental Treatment Facility. The Dental Assistant shall aid in the
provision of mandated dental surveillance and preventive services and
assist in ensuring the quality and timeliness of treatment records and
reports required to document procedures performed and care provided.
The Dental Assistant shall also refer patients who present with a
complaint to staff dentists for evaluation and continuation of care and
attend multidisciplinary treatment team meetings on behalf of the
dental officers. 2. Clinical Services required: the Dental Assistant's
clinical activity will be a function of the overall demand for dental
assisting services. The Dental Assistant's productivity is expected to
be comparable to that of other dental assistants assigned to the same
facility and authorized the same scope of practice. The Dental
Assistant shall: Provide for the examination, treatment, and
disposition of patients compatible with the Dental Treatment Facility's
operating capacity and equipment; Coordinate with other health care
departments and the clinic staff to provide complete care to patients;
Provide training to staff for routine dental assistant activities and
procedures so that the benefit of routine care can be accrued;
Maintain patient records in accordance with JCAHO and DTF
requirements;Select and arrange instruments and prepare set ups for
patient treatment; Assist during patient examination and treatment;
Assist during administration of anesthesia; Assist in placement and
removal of sutures; Prepare restorative and impression materials;
Dispose of contaminated waste in accordance with the standard
procedures of the DTF; Load and unload radiographic film cassettes;
Expose bitewing, periapical and occlusal film utilizing bisecting angle
or paralleling radiographic techniques;Perform radiographic darkroom
procedures to include manual and automatic film processing; Instruct on
basic oral hygiene care; and, maintain operatory to meet the clinic's
standards. The Dental Assistant may be assigned other duties consistent
with the normal duties of a dental assistant as directed by the
Commanding Officer. 3. Administrative and Training Requirements. The
Dental Assistant shall: Provide training and /or direction to
supporting government employees (technicians, corpsmen, students, etc.)
assigned to the contractor during the performance of clinical
procedures. Such direction and interaction will adhere to government
and professional clinical standards and accepted clinical protocol.
Participate in clinical staff quality assurance functions at the
prerogative of the Commanding Officer. The Dental Assistant may be
required to maintain statistical records of their clinical workload.
Participate in monthly meetings to review and evaluate the care
provided to patients, identify opportunities to improve the care
delivered, and recommend corrective action when problems exist. Should
a meeting occur outside of regular working hours, the Dental Assistant
shall be required to read and initial the minutes of the meeting.
Participate in the provision of monthly inservice training to
non-healthcare-practitioner members of the clinical and administrative
staff on subjects germane to dental care. Attend annual renewal of the
following Annual Training Requirements provided by the MTF: family
advocacy, disaster training, infection control, Sexual Harassment,
Bloodborne Pathogens and Fire Safety. Participate in the implementation
of the Hospital's Family Advocacy Program as directed. The Dental
Assistant is required to maintain American Heart Association CPR Health
Care Provider Course re-certification during the term of the contract.
Additionally, the contractor shall perform administrative functions
such as serving on boards and committees and attending or providing
continuing dental education. 4. Minimum Personnel Qualifications: To be
qualified for this position, contractor personnel must: 1. Have either
(a), a Certificate or Associate Degree as a dental
assistant/technician from a state accredited program and 12 months
experience within the preceding 36 months (b), certification from a
military dental technician or dental assistant "A" school and 12 months
experience within the preceding 36 months (c), certification from a Red
Cross Dental Assistant course and 12 months experience within the
preceding 36 months (d), 36 months experience within the preceding 60
months as a dental assistant in a private practice (e), membership in
good standing with the American Association of Dental Assistants with
required continuing education and 12 months experience within the
preceding 36 months or (f), graduation from a state accredited program
for dental assistants or dental technology within the preceding 12
months. 2. Have either certification or a permit for the use of dental
x-ray equipment. 3. Be eligible for U.S. employment. 4. Provide three
letters of recommendation, at least two of which must be from either
practicing dentists or faculty members where contractor personnel
received their dental assistant training. The letters must attest to
clinical skills, patient rapport, etc. Recommendation letters must
include name, title, phone number, date of reference, address and
signature of individual providing the letter. The third letter may be
from a previous employer. Reference letters must have been written
within the preceding five years. C. DENTAL HYGIENIST LOCATED AT
MIRAMAR: 1. The Dental Hygienist shall be responsible for the delivery
of treatment within the personnel and equipment capabilities of the
DTF, provision of mandated surveillance and preventive services, and
the quality and timeliness of treatment records and reports required to
document procedures performed and care provided. The contractor shall
direct supporting government employees assigned to them during the
performance of clinical procedures. Such direction and interaction will
comply with government and professional clinical standards and accepted
protocols. 2. Clinical Services required: Contractor clinical activity
will be a function of the overall demand for hygienist services.
Dental Hygienist productivity is expected to be comparable to that of
other dental hygienists assigned to the same facility and authorized
the same scope of practice. The Dental Hygienist shall: Provide oral
prophylaxis, preventive dentistry procedures and non-surgical
periodontal therapy to active duty military personnel and eligible
beneficiaries;Review and complete preliminary dental examinations for
new periodontal and recall patients. Oversee and manage periodontal
patient recall programs; Review patient's medical and dental history
for evidence of past and present conditions such as medical illnesses
and use of drugs which may complicate or modify dental hygiene
treatment; Examine teeth and surrounding tissues for evidence of
caries, periodontal disease and then record findings. Inspect head and
neck, examine mouth, throat and pharynx for evidence of disease such
as oral cancer and/or soft tissue pathosis; Expose, develop and
interpret radiographs to identify tooth structure, periodontal support
and other abnormalities such as periodontal bone loss, periapical
pathosis, caries, defective restorations, improper tooth contours and
contact relationships; Refer suspected medical conditions, hard and
soft tissue abnormalities, caries, periapical and periodontal pathosis
and traumatic or suspicious lesions to the dental officer for
evaluation;Perform pit and fissure sealant applications;Develop dental
hygiene treatment plans for patients including assessment of the
problem, type and extent of treatment required and sequence of
appointments to complete treatments;Perform complete oral prophylaxis
and non-surgical periodontal treatment on ambulatory patients using
ultrasonic and hand instruments; Perform subgingival scaling, root
planing and curettage under local anesthesia administered by dental
officer; if the appropriate background training and credentials exist,
administer local infiltration anesthesia;Treat acute necrotizing
ulcerative gingivitis; Polish teeth and apply disclosing solutions,
fluorides, desensitizing agents and other topical medications to the
teeth for the purpose of controlling caries and dentinal
hypersensitivity; Clean and polish removable dental appliances worn by
patients; Comply with applicable quality assurance standards for
preventive dentistry; Maintain a record of patient treatment and number
of patients treated; Record oral condition of teeth and supporting
tissues, type of therapy provided and progress notes; Clean and
maintain instruments and insure their sterility; clean and maintain
your work area to meet the clinic's standards; you may be assigned
other duties as directed by the Commanding Officer, consistent with the
normal duties of a dental hygienist. 3. Training and Patient education:
contractor personnel shall: Instruct patients, individually and in
group seminars, in proper oral hygiene using a variety of aids such as
models of teeth, slides, toothbrushes, floss, disclosing tablets,
mirrors, terproximal brushes and rubber tips; Plan and adapt oral home
care techniques to the specific need of the individual patient;
Explain causes of caries and periodontal disease to patients and the
importance of nutrition in maintaining dental and systemic health;
Monitor, supervise and assist in training dental technicians involved
in direct patient care to perform scaling, prophylaxes, polishing
procedures, fluoride applications and oral home care instructions. 4.
Minimum Personnel Qualifications: To be qualified for this position
contractor personnel must: 1.Have a degree or certificate in dental
hygiene from a school of dental hygiene OR a doctorate in dentistry
approved by the Council on Dental Education of the American Dental
Association (ADA). 2. Hold a current, unrestricted license to practice
dental hygiene or general dentistry in any one of the fifty States,
the District of Columbia, the Commonwealth of Puerto Rico, Guam or the
U.S. Virgin Islands. 3. Either (a), successfully complete at least 12
classroom hours of continuing dental education within the preceding 18
months which maintain skills and knowledge in dental hygiene and
preventive dentistry, or (b) graduate from an ADA approved dental
hygiene OR general dentistry program within the preceding 12 months. 4.
Have experience as a Dental Hygienist OR General Dentist of at least 12
months within the preceding 24 months, unless a recent graduate. 5. Be
eligible for U.S. employment. 6. Provide three letters of
recommendation from practicing dentists attesting to contractor
personnel clinical skills. Letters of recommendation must include name,
title, date of reference, phone number, address and signature of
individual providing reference and must be written within the preceding
5 years. Recent graduates may provide letters of recommendation from
faculty where dental hygiene or general dentistry training was

DFARS 252.212-7001, Contract Terms and Conditions Required to
Implement Statutes or Executive Orders Applicable to Defense
Acquisitions or Commercial Items, applies with the following clauses
applicable for paragraph (b): DFARS 252.225-7001, Buy American Act and
Balance of Payments Program; DFARS 252.225-7014, Preference for
Domestic Specialty Metals; and DFARS 252.225-7012, Preference for
Certain Domestic Commodities. OFFERORS ARE REQUIRED TO COMPLETE AND
INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPOSALS: FAR
52.212-3, Offeror Representations and Certifications -- Commercial
Items, FAR 52.209-5, Certification Regarding Debarment, Suspension,
Proposed Debarment, and Other Responsibility Matters. The Government
intends to make a single award to the responsible offeror whose offer
is the most advantageous to the Government considering price and
price-related factors. Provision 52.212-2, Evaluation -- Commercial
Items, applies with paragraph (a) completed as follows: The following
factors shall be used to evaluate offers: 1. Minimum Personnel
Qualifications, 2. Enhancing Criteria, 3. Price. Offeror's proposals
will be evaluated first under Factor (1) above, which represents
minimum acceptable criteria. Failure to meet the minimum criteria for
this factor will render the offer technically unacceptable and shall
not be considered for award. Only those offerors who meet the Minimum
Personnel Qualifications required for the Dental Assistants and Dental
Hygienist (listed in the Statement of Work) will be further evaluated
under Factors (2) and (3). Factor (2) shall be evaluated by ranking
the offeror against all other technically acceptable offerors using the
following Enhancing Criteria, listed in descending order of importance:
a. Experience, both quality and quantity, as a Dental Hygienist or
Dentist, and Dental Assistant. b. The letters of recommendation
required above, may enhance the contractor's ranking if they address
such items as clinical skills, professionalism, or specific areas of
expertise, etc. c. Total Continuing Education hours. d. Infiltration
Anesthesia certification. e. Additional Dental/Medical certifications.
f. Current American Heart Association Basic Life Support (BLS) for
Healthcare Providers, American Heart Association Healthcare Provider
Course; American Red Cross CPR (Cardio Pulmonary Resuscitation) for the
Professional Rescuer; or equivalent. g. Prior military experience in a
Dental/medical field (provide Form DD214). Factor (2), Enhancing
Criteria, when considered in its entirety, is approximately equal to
Factor (3), Price. To be determined technically acceptable under Factor
(1), the offeror shall furnish to the Government along with their offer
copies of all certificates, degrees, permits, licenses, experience,
letters of recommendation, memberships, and class room hours for all
