This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. This is a simplified acquisition, issued as a Request for Quotation (RFQ), number SSA-RFQ-99-0060. The prospective quoter may quote either a GSA Schedule Contract Price or an Open Market Price. If quoting on a GSA Schedule Chair, the prospective quoter shall cite both their GSA Contract Number and the Special Item Number (SIN) that the chairs fall under. If quoting an Open Market Chair, the prospective quoter shall state the fact that their chair is an Open Market Chair. This request for quotations and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-12. If quoting on an Open Market Chair only, this requirement is 100% set-aside for small businessesonly. If quoting on a GSA Schedule Chair, any size business may submit a quote. All responsible sources may submit a quote which shall be considered by the agency. The standard classification code for this requirement is 5044. REQUIREMENT: The Social Security Administration in Baltimore, MD requires delivery of the following chair: Chair, Executive Low-Back, 23"W x 20"D x 30-35"H, 47 lbs., sort foam padding for lumbar support with extra support in the hip area. Executive models have swivel/tilt mechanism, upholstered arms, Frame Color: Black, Fabric Color: Burgundy, Model Number: 653-322C, Requested Quantity: 275 each. This acquisition shall be solicited on a Brand Name "Or Equal" basis. If quoting on the brand name, please cite the brand name together with a picture of the chair being proposed. If quoting on an "Or Equal" chair, please cite the model number of the chair being quoted on together with a picture of the chair being proposed. All chairs shall be shipped and all quotes shall be submitted on an F.O.B. Destination basis, unless the prospective quoter is quoting on a GSA Schedule chair where the F.O.B. point is already set. The ship-to address is Social Security Administration, Attn: Receiving Clerk, 300 North Greene St., Baltimore, MD 21290-0300. In its quotation, the quoter shall include the following information: business size; does the quoter accept the Government bankcard; prompt payment terms if the quoter does not accept the Government bankcard; time of delivery in calendar days; federal tax number; DUNS number; company name, address and phone number as it shall appear on the award document. The Government's purchase order number, 0440-99-38307, must appear on all boxes, cartons and containers delivered. BASIS FOR AWARD: Award will be made to the quoter whose quote represents the technically acceptable, lowest overall total quoted price to the Government. The Government shall make only one (1) award. The provisions at FAR Part 52.212-4, Contract Terms and Conditions Commercial Items apply to thisacquisition. There are no addenda to these provisions.
