HQ-99-06 previously posted in the CBDNet on August 25, 1999 has been amended. Amendment A00004 is issued to the above combined synopsis/solicitation for purchase of Employee Assistance Program services. This amendment is issued in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes an amendment of the above solicitation, proposals are being requested, and a written amendment will not be issued. This amendment is issued to furnish questions and answers concerning this synopsis/solicitation AND TO EXTEND THE DUE DATE. QUESTIONS AND ANSWERS: Question No. 1: What was the utilization rate for services in 1998? Answer: 11.4%. Question No. 2: If INS exceeds the number of assumed hours/referrals, who is responsible for the overage. Answer: In unusual circumstances, the Contractor may seek additional sessions. The Contractor must write a relevant clinical argument justifying the need for additional sessions to the EAP Manager. Extensions will be granted when it is deemed an emergency, the employee has no mental health insurance, the employee's insurance does not cover the counseling required, or when it serves the best interest of the Government. The EAP manager will approve/disapprove in writing such requests and assign a control number. The Contractor shall reference the control number on subsequent vouchers/invoices to receive payment for additional sessions. Question No. 3: Is there a limit to the number of sessions for the EAP counseling services. Answer: Short term counseling and all referrals shall be provided in 3 sessions or less. If additional sessions are needed the procedures addressed in the answer to Question 2 above must be followed. Supervisor and Management consultations are limited to 6 sessions. Question No. 4: Is INS open to a capitated pricing arrangement? Answer: INS requires that pricing be submitted in the format identified for the line items in the combined synopsis/solicitation. Offerors may submit capitated rate information as supplemental pricing information at their own option. Question No. 5: Is INS requesting a 2 year rate guarantee? Answer: Yes, offerors should factor in any increases required in the option year pricing. It is at the offerors discretion as to rates proposed for the base year and option year. Question No. 6: Are the assumed hours for Employment Services, Vocational Interest Tests, and Mediation based on actual utilization? Answer: The hours for Employment Services, Vocational Interest Tests, and Mediation are based on anticipated need. THE DATE FOR RECEIPT OF PROPOSALS IS HEREBY EXTENDED TO OCTOBER 5, 1999, 3:00 P.M., LOCAL (WASHINGTON, DC) TIME. No further extensions in the proposal submission date will be granted due to offeror questions. Offerors may continue to submit questions and answers will be posted in the Commerce Business Daily (CBD). AMENDMENT ACKNOWLEDGEMENT: Proposals must acknowledge receipt of this amendment before the hour and date specified in the synopsis/solicitation. Failure of this acknowledgement to accompany the proposal may result in rejection of your proposal. If as a result of this amendment you desire to change a proposal already submitted, such change may be made by a letter. The letter must refer to the RFP No. HQ-99-06 and this amendment, and be received before the time and date specified. The synopsis/solicitation HQ-99-06 is amended as indicated above. The hour and date for receipt of proposals is extended to October 5, 1999, 3:00 p.m. local (Washington, DC) time.
