The Portland VA Medical Center requires commercially available Home Health Care Services to patients referred by the Department of Veterans Affairs in accordance with all the terms and conditions of the contract. Services will be performed in the local Portland, Oregon and Vancouver, Washington areas and surrounding counties. Offerors must be accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) according to Home Care Standards, Medicare certified and licensed by the state(s) in which service will be provided. Services provided will include the following 6 disciplines: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work, and Home Health Aid. This contract will delineate reimbursement rates when home health services are paid directly by PVAMC. Services shall be provided for the period beginning December 1, 1999 through November 30, 2000, with two (2) options to renew for a period of one (1) year each.
