Q -- OCCUPATIONAL HEALTH EXAMINATIONS SOL 00-SQ-34-0081 DUE 120699 POC Larry Miller, Contract Specialist -- Tel 520-343-8137 or Fax 520-343-8568 Furnish comprehensive physical examinations for selected employees of the Bureau of Reclamation, Yuma Area Office, Yuma Arizona. Examinations are to determine the employees's ability to perform the scope of work of the occupation to which assigned. Examinations are either Fitness for Duty or Hazardous Waste Workers. Examinations must comply with the Occupational Safety and Health Administration Guidelines. Exam results will be forwarded to a Medical Review Officer of the Federal Occupational Health Program. Contractor must provide written evidence of Arizona medical license, certification of clinical staff, lab and lab equipment must be certified. The contractor must also provide evidence of professional liability insurance. In an effort to reduce the amount of employee down time for accomplishment of required examinations, the Government is restricting the area of consideration to the Yuma, Arizona Metro Area. Examinations should be completed in a single visit to the contractors facility. The Government intends to make this acquisition under the provisions of FAR Part 12. The contract is for a base year with two option periods. All responsible sources may submit a quote which shall be considered by the agency.
