synopsis/solicitation. Request for Quotation #RFQ 618-83-00 entitled
"Psychiatric/Addiction Services" is hereby issued for commercial items
to be used by the VA and is prepared in accordance with the format in
Federal Acquisition Regulation (FAR) Subpart 12, Acquisition of
Commercial Items, Subpart 12.6 and Part 13, Simplified Acquisition
Procedures and as supplemented with additional information included in
this notice. Simplified acquisition procedures apply and the VA will
conduct the procurement according to a combination of Parts 12 and 13.
Solicitations are being requested under Public Law 104-262 The
Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United
States Code Section 8153, with a Commercial Items format of terms,
conditions, and provisions/clauses. This procurement is being conducted
under Federal Acquisition Regulation (FAR) Subpart 13.5 test program;
simplified acquisition procedures apply. This CBD announcement
constitutes the only solicitation issued. A separate written
solicitation document will not be issued. Incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-14
and VAAR 97-7. The Standard Industrial Code (SIC) is 8062 and the
small business size standard is $5 million. Contractor's proposals
(cost and technical) are herein requested. A response to requirements
listed below under a firm fixed price contract shall be submitted on
contractor's letterhead or formal quotation form and must address all
requirements listed in a separate technical proposal. Contractor's
price and technical proposal must contain the name; title and signature
of person authorized to submit proposals/bids on behalf of the
contractor. Provide Board Certified, unrestricted license in U.S.
(certification in addictions preferred), Psychiatrist Services to and
at VA Medical Center (VAMC), Minneapolis, MN for an estimated six-month
period beginning January 3, 2000 and ending June 30, 2000. Services
include but are not limited to the following: Provide
psychiatric/addiction services to dual disordered patients in a team
environment. Will staff approximately 20-28 direct clinic hours/week,
plus team meetings and other duties as assigned. Tour will be 5-days
per week coverage (40 hours per week) 8:00 a.m. to 4:30 p.m.
Contractor's employees must maintain the highest standards of integrity
and forthrightness during interactions with patients, nurses, other
physicians, and other Health Care Professionals. It is essential that
the contractor's personnel be able to effectively interact with a
variety of individuals. Contractor's employees must ensure compliance
with all applicable JCAHO Guidelines. Comply with appropriate and
applicable policies, procedures and regulations of the Minneapolis VA
Medical Center. A full-time equivalent (FTE) is defined as a
contractor's employee working eight (8) hours per day, five (5) days
per week (40 hours per week). Contractor's employees must be present at
VAMC and must be actually performing the required servicesfor the
period specified in the contract or the contract cost will be decreased
accordingly during each billing cycle. A VAMC record-keeping system of
contractor hours worked is established through time and attendance
logs in the departments being served. The payments to contract
personnel for any leave, including sick leave, holiday, or vacation
time, is the responsibility of the contractor. Leave granted to
contract employees would be in accordance with contractor policies. VA
is not obligated to the Contractor during physician absence.
Evaluation of proposals offered will include the following factors:
Professional qualifications, clinical and consultation
skills/interpersonal skills, availability, past performance and cost.
Award will be made to the offeror determined to be the "best value" to
the Government. Evaluation may be made with or without discussions
with the offeror(s). All responsible sources that can meet the
requirements and provide services as listed above may respond to this
solicitation by submitting cost and technical proposals (curriculum
vitae) on their company letterhead or bid form. The following
provisions and clauses shall apply to this solicitation: 52.252-2
