Q -- PHYSICAL THERAPIST SERVICES SOL NHDC-00-P-0006 POC Suzanne Shumate, (225)642-4704 Services of a Physical Therapist from 8 AM to 4:30 PM Monday through Friday except Federal holidays to perform PT treatments on patients with Hansen's Disease. Period of performance: April 1, 2000 through March 31, 2001, with two one year options. Therapist shall be licensed, have excellent ability to speak, read and write English and ability to work in a team environment. Therapist shall have a minimum of 5 years experience with wound care which includes neuropathic foot care and 3 years experience with amputee rehabilitation, including prosthetics fitting and gait training. Duties include treatment of patients with neuropathic limb dysfunction secondary to Hansen's Disease including foot screening, wound dressings, compression pumps and assisting in fabrication of splints, casts, foot models, foot orthoses and modifications of footwear; and performance of musculoskeletal and ADL services including performing physical therapy modalities, range of motion and strengthening exercises and ambulation training.
