65 -- INSTALLATION OF SOUND BOOTH) SOL RFQ 600-053-00 DUE 050500 POC Aaron Villalpando, Contracting Officer, (562) 494-5939, Fax (562) 494-5828 COMBINED SYNOPSIS/ SOLICITATION IN CBD IAC Medical/Life Sciences Examination and Research Rooms Model 1203A RFQ 600-053-00 Scope of Work: This is a combined synopsis/solicitation for commercial items prepared in accordance with subparts in the Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-15. The Government intends to procure an Industrial Acoustics Company, Medical/Life Sciences Examination Room (Sound Booth), Model No. 1203A, in this Solicitation No. 600-119-00, from Audiometrics on a Brand Name or Equal basis (SIC 3845). The Sound Booth is a double-wall (room within a room) construction. Two housings constructed of 4 inch (102mm) thick modular panels, separated by a 4 inch (102mm) air space. Inner room floor floats on vibration isolators; outer wall rests on area floor or vibration-isolated floor. The Sound Booth shall be constructed of Fire-Noise-Lock Panels fire rated by U.L. 90 minutes with sound absorptive surfaces facing fire inside room. Rating for solid surfaces facing fire outside room shall not be less than 45 minutes. Each Fire-Noise-Lock shall bear a label with Listing Mark of Underwriters Laboratories, Inc. certifying dual ratings. The inside dimensions are: width 7'-4" (2236mm), length 7'-0" (2135mm), height 6'-6" (1980mm). The outside dimensions are: width 9'-4" (2845mm), length 9'-0" (2745mm), height 8'-1" (2464mm). The contractor shall provide proof that he has trained personnel and his own shop facilities for performing the work under established quality-control procedures. Offeror shall propose pricing for the Medical/Life Sciences Examination Room. Included in the pricing are the following line items: 1) Sound Booth which will include shipping and handling. 2) Installation of the Sound Booth. The Contractor warrants that at the time of delivery, the system will be free from defects in material and workmanship and will be in good working condition. Delivery of the Medical/Life Sciences Examination Room, Model 1203A shall be FOB Destination to the Department of Veterans Affairs, North County Clinic, 1830 W. Drive, Suite 107, Vista, CA 92083. The required date of delivery is June 19, 2000. The installation must be completed by June 27, 2000.
