is amended to (1) extend the closing to 11 may 2000;and (2) add the
following safety requirements under which the contractor will be
required to perform these services: The work is to be performed at the
cold regions research and engineering lab, 72 Lyme road, Hanover, NH.
This is a government facility and all work will be performed in
accordance with applicable provisions of u.s. army corps of engineers
safety and health requirements manual, em 385-1-1, available at CRREL
for review, and OSHA 29 CFR 1910 regulations. All applicable state and
federal safety regulations relating to protection of eyes, other
extremities, and hearing protection shall be followed. Machinery and
equipment shall be operated in accordance with the manufacturer's
instructions and recommendations. Material safety data sheets are
required for any products or materials brought on the facility for use.
All contractor employees are required to sign in at the reception area
located in the front entrance to the mainlaboratory building. Work
will be performed during normal working hours between 7:00am and
5:00pm, Monday through Friday. Additional hours may be arranged with
prior approval from the CRREL point of contact. The contractor shall be
responsible for any damages that may occur to CRREL property while
performing the service of this contract. Ice engineering facility --
the anhydrous ammonia refrigeration system within the ice engineering
facility falls under the OSHA process safety management (PSM) standard,
29 CFR 1910.119. Contractor's performing maintenance or repair,
renovation or specialty work on or adjacent to the ammonia plant
process are required to comply with these guidelines. This will assure
CRREL that the safety and health of the employees of the facility is
not compromised. The contractor will fill out the form, contractor
qualifications and reference questionnaire (cq-1). This form must be
requested from this office. CRREL's PSM team will evaluate the
completed form. The contractor must alsoprovide the following
documentation for review -- (1) the contractor must supply written
records of past safety performances; (2) the contractor must supply
documentation that each employee has been trained in the work practices
necessary to perform their jobs safely. This includes instruction in
the known potential fire, explosion, or toxic release hazards related
to the job and the applicable provisions of the emergency action plan.
The documentation must identify each employee by name and date of
training. This will be in the form of certified 24-hour HAZWOPER
training per 29 CFR 1910.120(q); and (3) the contractor must supply
certification of employee training in lock-out/tag-out procedures, and
hot work permit procedures, respiratory protection plan and fit test
certification (certification of medical clearance). Prior to start of
work a pre-job conference will be held involving the contractor and the
process safety management team to review the following -- (1) CRREL's
emergency response planand site evacuation plan; (2) contractor's
emergency communication system; and (3) contractor's method for
determining ammonia concentration levels. A work sequence plan in the
form of a risk analysis plan is required prior to the start of any
