COMMERCE BUSINESS DAILY ISSUE OF JULY 7,2000 PSA#2637

P -- LOADING AND REMOVAL OF PAVEMENT DEBRIS This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a formalized written solicitation will not be issued. This solicitation, F05604-00-R-8004, is being issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular 97-13. The Standard Industrial Classification (SIC) code is 4953 and the Business Size Standard is $6.0M. The proposed contract will be solicited as "100 percent small business set-aside". STATEMENT OF WORK FOR LOAD AND REMOVAL OF PAVEMENT DEBRIS 1. GENERAL INFORMATION. The Contractor shall provide all management, tools, supplies, equipment and labor necessary to ensure that debris removal services are performed at Peterson AFB Colorado in a manner that will comply with government and commercial standards for debris removal. Estimated quantities are: 1.1. ROUTINE SERVICE CALLS. The government estimates approximately 4,000 tons of debris on an on-call basis per contract year. 1.2. SPECIAL EVENTS SERVICE CALLS. The government estimates two (2) special events service calls per contract year. Each load is estimated to contain anywhere from 20 to 2,400 tons of debris. 2. SUPERINTENDENCE BY CONTRACTOR. The contractor shall at all times provide adequate supervision of his employees to ensure complete and satisfactory performance of all work in accordance with the terms of the contract. 3. SPECIFICATIONS AND DESCRIPTION OF WORK. 3.1. ENVIRONMENTAL AND HAZARDOUS MATERIALS HANDLING. If the Contractor suspects there may be any hazardous materials mixed in with the debris, he shall notify the QA PERSONNEL immediately and await further instructions. 3.2. RECORDS. The contractor shall maintain sufficiently detailed records to indicate the dates of waste pick-ups, the method of transportation, the segregation or commingling of different types of wastes, and the location of the disposal site. If waste has been diverted to a recycling or a reuse effort the contractor shall provide the Environmental office with an accurate weight report. The contractor shall provide this information to the Base Civil Engineering Environmental Management and Planning Flight within five (5) working days of receipt of the request. 3.2.1. The Contractor shall send a copy of the receipt from the landfill or recycling center stating the number of cubic yards (or provide weight tickets) of debris disposed from Peterson AFB with his invoice. Air Force Diversion Rate and Reporting Requirements. The contractor shall provide the following demolition project information: 3.2.1.1. Amount (tons) of demolition debris solid waste recycled reused or diverted from landfill. 3.2.1.2. Amount (ton) of construction/demolition non-hazardous solid waste sent to landfill facilities. 3.3. LICENSES AND PERMITS. The contractor shall be responsible for obtaining any necessary licenses and permits, and for complying with any federal, state, and municipal laws, codes, and regulations applicable to the performance of work. Contractor shall provide proof of such applicable licenses and permits upon request of the Air Force. 3.4. ROUTINE SERVICE CALLS. The Contractor shall load and remove from Peterson AFB all pavement debris to include concrete, asphalt, rocks, dirt, debris, wood and metal WITHIN FIVE (5) WORKDAYS after notification by the Quality Assurance (QA) personnel. All debris removed shall be properly disposed of off Peterson AFB by the Contractor at a suitable, approved landfill or recycling center in compliance with all applicable federal, state, and local regulations. 3.5. SPECIAL EVENTS SERVICE CALLS. On occasion, there may be a requirement to remove pavement debris due to a VIP visit, IG inspection, etc. The QA personnel will notify the contractor as soon as a special event requirement is known, but not less than 48 hours prior to the event to provide as much notification as possible. The contractor shall load and remove from Peterson AFB all pavement debris to include concrete, asphalt, rocks, dirt, debris, wood and metal WITHIN TWO (2) WORKDAYS after notification by the QA personnel. All debris removed shall be properly disposed of off Peterson AFB by the contractor at a suitable, approved landfill or recycling center in compliance with all applicable federal state and local regulations. 4. QUALITY CONTROL. The contractor shall develop and maintain a quality program to ensure services are performed in accordance with commonly accepted commercial practices. Contractor shall develop and implement procedures to identify and prevent defective services from recurring. 5. QUALITY ASSURANCE. The government will periodically evaluate the contractor_s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor_s performance through intermittent on-site inspections of the contractor_s quality control program and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if performance dictates. The government will also receive and investigate complaints from various customers located on the installation. The contractor shall be responsible for initially validating customer complaints. However, the government representative shall make final determination of the validity of customer complaints(s) in cases of disagreement with customer(s). 6. HOURS OF OPERATION. 6.1. NORMAL HOURS OF OPERATION. The contractor shall provide services required under this contract during the following hours: 7:30 a.m. and 4:00 p.m. Monday through Friday. 6.2. HOLIDAYS. Work shallnot be required on the following holidays or any holidays observed in lieu thereof: New Year's Day (January) Labor Day (September) Martin Luther King Day (January) Columbus Day (October) President's Day (February) Veterans' Day (November) Memorial Day (May) Thanksgiving Day (November) Independence Day (July) Christmas Day (December) 7. SECURITY REQUIREMENTS. The Contractor shall be responsible for obtaining all necessary cards, passes, decals or other items required for access to Peterson AFB. The Contractor shall be required to apply for vehicle and individual identification at the Peterson AFB Security Police Squadron Visitor's Control Center, Bldg 1820. All personnel operating a vehicle on Peterson AFB shall be a licensed driver and all vehicles shall be licensed and insured. Upon completion or termination of the contract, the Contractor shall surrender any vehicle identification to the Security Police at the Visitor's Control Center, Bldg 1820.
