This is a combined synopsis for commercial items prepared in accordance with FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation RFQ 552-25-00. The Standard Industrial Code is 3826, and the small business size is 500 employees. This solicitation document provides incorporated clauses that are in effect through Federal Acquisition Circular 97-05. The Dayton VAMC is interested in purchasing a Methadone Inventory and Dispensing System which includes software package with support and upgrades for physician module, administration module financial mondule, prescription module and all associated equipment, installation and training. All responsible sources may submit a written firm fixed price proposal with adequate technical information to evaluate the system. Evaluations will be made in the following order of importance with price being equal to 50%: Technical capability; References and contacts for current users of the system; and price. Technical capability shall include all technical specifications, product availability, product picture, product features, etc, incomplete technical data will be rejected. Quotations shall be submitted on an "all or none" basis. Offers shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offerors Representation and Certifications -- Commercial Items.
