Description

Reference to announcement number 496971 dated September 14, 2000 for Positron Emission Tomography (PET) Furniture for Nuclear Medicine Hot Lab has been amended. Amendment number 2 is as follow: Area Hot Lab. Number (1) one with: (1) one Preparation and Fume Hood Enclosure (36" W x 26.5 "D x 33.5 "W and 1/2" thick lead), (1) one Enclosure Base Module (36.5" W x 30" D x 36.5" H and 1/2" thick lead), (1) one Sink and Decay Storage Module {30.5" W x 30" D x 36.5" H and 1/2" thick lead), (1) one 12 Drawer Storage Module for Pet (32"W x 30" D x 36"H with 2" thick lead), (1) Generator and Storage Module (36.5" W x 30"D x 36.5" H and 1/2" thick lead). Modules will sit side-by-side with a single stainless steel counter top covering all (5) five units and will be 171.5 inches long and includes (1) one sink. Area Hot Lab. Number (2) two with: (1) one Enclosure Base Module (36.5" W x 30" D x 36.5" H and 1/2" thick lead), (1) one Generator and storage module (36.5" W x 30" D x 36.5" H and 1/2" thick lead), (1) One Unit Dose Module (30.5" W x 30" D x 36" H and 1/2" thick lead). Modules will sit side-by-side. A single stainless steel counter top will cover all (3) three units and will be 103.5 inches long. Accessories Hot lab. Number (3) three with: (1) one Pet Deluxe L-Block shield, (1) one Positron Chamber Shield for CRC- l5R Dose Calibrator, (1) one Angel Shield 3 cc, (1) one Angel Shield 5cc, (1) one Angel 5hield 10cc, (1) one Angel Shield Stand 3cc to l0cc with Secondary Shield. FOB Origin. After contract award the Contractor shall submit packing information to include dimensions (H x W x L), weight for each box, number of pieces and shipping point. The quotations must be received NLT 4:00pm (local time) on 09/27/00. All the other terms and conditions remain unchanged.
