Description

The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Quotation (RFQ) in accordance with FAR 12.203 to provide Durable Medical Equipment for the Department of Veterans Affairs, Central California Health Care System, 2615 E. Clinton Avenue, Fresno CA 93703, a Department of Veterans Affairs facility within the Veterans Integrated Service Network (VISN 21). The contractor shall furnish, deliver, set-up, recover, clean, store, reissue, new and used medical equipment and supplies to U.S. Department of Veterans Affairs (VA) beneficiaries' homes or to the VA. Contract period is February 1, 2001, or date of award, through January 31, 2002 with four one-year renewal options. Contract award is subject to availability of funds. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors Commercial Items; 52.212-2 Evaluation Commercial Items; 52.212-3 Offeror Representations and Certifications -- Commercial Items; 52.212-4 Contract Terms and Conditions Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders Commercial Items. NAICs Code is 421450. Offerors shall submit proof of current JCAHO accreditation.
