Description

RFP 514-8-01 will be issued on or about January 24,2001 for the provision of services at a location in the County of Yates in the State of New York. The Veterans Affairs Healthcare Network Upstate New York (VAHNUNY) desires to establish a contract for Community Based Primary/Preventative Care services for veterans in this geographic area. The resulting contract will be for a period of one year with two options to commence on or about June 1, 2001. The Contractor will be responsible to furnish medical facilities, clinical providers, equipment, supplies, routine radiology, space for VA staffed Mental Health Services and administrative functions to fulfill the level of care specified in the RFP. Service will be at a fee-for service rate for an estimated 900 uniques per year. The NAICS code is 621498 and the Commercial Items solicitation is unrestricted.
