Description

Contractor shall provide for the delivery and management of primary and preventive medical care and the continuity of that care to eligible beneficiaries of the Department of Veterans Affairs. The eligibility of veterans will be determined by the John D. Dingell VA Medical Center, Detroit, Michigan and confirmed in writing to the provider. The contractor shall be centrally located in the Yale, Michigan area to serve veterans anticipated to receive primary care services who reside in the areas partially designated as Medically Underserved Areas/Medically Underserved Population and/or Health Professional Shortage. Interested providers are to request the solicitation in writing, no electronic request will be accepted. Requests are to be submitted on company letterhead to include telephone number, telefax number and point of contact. Solicitation will be issued on or about 3/30/01. Award is subject to the availability of funds.
