DESCRIPTION: Non-personal service to provide all personnel, equipment, facilities, supplies, services and materials for the purchase of Cellular Phones and Service for F. E. Warren AFB, WY and surrounding tri-state area to include Cheyenne, WY, areas north and west of Cheyenne, Northern Colorado state line to Colorado Springs, CO and Western Nebraska. The contract period will be 01 Apr 01 through 30 Sep 01 with two (2) option years, CLINS 0001 through 0003. Faxed or telegraph offers will not be accepted. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested and written solicitations will not be issued. Solicitation F48608-01-R0003 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-16. (FAR Clauses may be found by accessing WEB site at: farsite.hill.af.mil. North American Industry Classification System (NAICS) code and small business standard or this acquisition is 513322. The solicitation is open to Large and Small Businesses. Anticipated award date is 9 Mar 01. The provisions at FAR 52.212-1, Instructions to Offer(s) -- Commercial Items; FAR 52.212-2 Evaluation-Commercial Items; FAR 52.212-3, Offeror Representations and Certifications_Commercial Items, apply to this acquisition. A completed, signed copy to this provision FAR 52.212-3 shall be submitted with any offer. The clause at FAR 52.212-4, Contract Terms and Conditions applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. Other FAR clauses are incorporated by addendum at Attachment 3. Evaluation will be by Performance Price Trade-Off (PPT) and past performance will be evaluated.
