Description

The National Hansen's Disease Programs is conducting a market survey to determine the existence of businesses capable of providing independent living/assisted living services for approximately 35 HD patients. THIS IS A MARKET SURVEY ONLY. THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. The potential services include labor, facilities, materials, supplies and transportation to provide independent living and assisted living for approximately 35 HD patients at a single location. The potential requirements include living facilities, meal and nutrition service, recreation service, and patient assistance services (such as bathing, housekeeping, medication dispensing, laundry, etc.). Facilities must meet state and federal requirements governing such facilities. The geographical zone of consideration is the metropolitan Baton Rouge LA. Interested and qualified organizations within the geographical zone of consideration, with a demonstrated past performance providing these particular types of services are requested to provide a statement of current capability and past performance information. Sources are requested to provide a description of their current capabilities, including but not limited to current space available, amenities and/or demonstrate plans to obtain the resources necessary provide these requirements. Interested parties are requested to provide the following information: (1) Company name, point of contact, physical address, e-mail address, telephone and facsimile numbers, and type and business size; (2) Current space available and planned expansions; (3) Amenities and services currently provided or planned to provide. (4) Age of the facility (5) Name of principals to contact, including title and telephone number. (6) Current price list. This synopsis is for the sole purpose of identifying potential sources and does not commit the Government to issuing a solicitation or making an award. Interested parties should provide this information by February 9, 2001. All information submitted is at the respondent's own expense. The Government will not reimburse respondents for any cost associated with submission of capability statements in connection with this market survey. Respondents will not be contacted regarding their submission of information gathered as a result of this notice. Any information provided is voluntary.
