- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes the only document to be issued. A formal solicitation WILL NOT be issued. This Notice incorporates provisions and clauses in effect through Federal Acquisition 97-19. RFP 550-06-01 should be referenced on the proposal. The North American Industry Classification System(NAICS) code for this procurement is 621999 and the small business size standard is gross annual receipts NTE $7.5 million. This procurement is unrestricted. Requirement: The VA Medical Center, 1900 East Main Street, Danville, IL 61832, has an emergent need for a Board Certified Gastroenterologist Physician to provide GI and management services to veteran patients and eligible beneficiaries. Services shall be required for 24 hours per week (3 days per week), 8:00 a.m. to 4:30 p.m., excluding holidays. Danville VAMC, intends to solicit proposals and award a firm, fixed price contract under the authority of Title 38 USC Section 8153. The contract is for a period of 7 months (anticipated through September 30, 2001). The VA reserves the right to reduce the term of the contract with a 30 day notice if recruitment efforts prove successful. The Gastroenterologist shall be a citizen of the United States and shall hold a valid and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or the District of Columbia and shall be proficient in the English language, both spoken and written. The Gastroenterologist shall perform the following functions at this facility: (1) Provide inpatient consultations in gastroenterology; (2) Conduct general hepatology and gastroenterology outpatient clinic services; (3) Be responsible for appropriate documentation of clinic notes, consultations, and procedure notes; (4) Perform routine/emergency diagnostic and therapeutic Endoscopy lab procedures, including but not limited to: Conscious sedation; Esophago-Gastroduodenscopy; Colonoscopy; ERCP; Liver biopsy; Paracentesis; and Percutaneous Endoscopic Gastrostomy tube placement. The clauses at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May 1999) applies to this acquisition. There is an addendum to this clause. Add the following clauses: FAR 52.252-2, Clauses Incorporated by Reference (Feb 1998), This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Award will be made to the offeror whose proposal conforms to the requirements of the solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award will made be made without discussions.
