Description
Contractor shall provide to the Department of Veterans Affairs, VA Gulf Coast Veterans Healthcare Systems, audiometric equipment. This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number/Request for Quotes (RFQ) 520-01-13 is assigned and shall be referenced on any quotation. This is a Request for Quotation for which is and all incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-21. This is 100% set-aside for small business. The NAICS will be 42145. The small business size standard for this procurement is 100. This RFQ is being solicitated on a "brand name or equal" basis. Offerors must submit brand name, model, and warranty for each unit being offered. NOTE: Descriptive literature: Offerors are cautioned that the responsiveness of their quotation depends on the completeness and sufficiency of the descriptive material submitted with the quotation. Bidders must highlight descriptive literature to show the items meet the minimum requirements. The contractor shall provide audiometric equipment as follows: Complete Digital video Stroboscopy System Model 9295 OR EQUAL to include at a minimum: a. Stroboscope endoscope light source which consists of halogen (constant) and zenon(flash) light sources; laryngeal and audio microphones; pitch triggering by laryngeal microphone or EGG input; digital display of pitch, phase and amplitude measurements; four (4) modes of operation (slow, fast, locked and manual); foot switch and pedal for remote control of instrument functions; and isolation transformer. b. Rigid endoscope (70 deg.) c. Three-chip camera with three inch CCD sensors with DSP, 700 line resolution, field/frame integration, 4 lux sensitivity. d. 3.mm flexible endoscope with 120 degree up/down tip deflection. Can be used with adults or children. e. 35mm Lens/adapter f. Digital Video Computer System with hardware and software for recording examinations and interfacing to camera, light source and printer. Features: Computer (Pentium III 550 MHz, PCI Ultra-wide SCSI architecture, 2X 18 GB hard disk, 128 MB RAM, 20x CD-ROM, modem, with WinNT 4.0 operating system) Internal DVD-RAM recording system, 4GB per cartridges 19" Super VGA monitor Professional level (RGB component recording) video capture board with MJPEG compression daughterboard Hardware and software for computer control of camera white balance setting Software for patient and examination database management, and image capture, processing and playback Electroglottograph acquisition and display (requires electroglottograph input) g. Flexible Scope adapter (Olympus) h. DVD Ram Disks 4GB storage capacity (10 Disks) i. Factory authorized person to set up system and provide day of training. j. Color printer (digital) which uses 8 x 11" paper. Suitable for reports and image printing. k. Cart (14 gauge steel) custom for system l. Electroglottagraph (needed for EGG display) Salient Characteristics on Digital Video Strobobscopy System, Model 9295 as follows: 1. DVD-RAM drive must store up to 100 minutes of digitally recorded exams per disks. 2. CCD writer allows images to be transmitted to referring physicians. 3. Report format and patient data storage system Digital Swallowing Workstation Model 7200 OR EQUAL to include at a minimum: a. Xenon Light source constant xenon light source with adjustable brightness b. CCD Camera CCD camera with inch CCD sensor, S-VHS; variable AGC, power pack, and C-mount, 5 lux sensitivity, including cables. c. Flexible endoscope 3.5mm flexible endoscope with 120-degree up/down tip deflection. Can be used with adults or children d. Lens adaptor 35mm lens with eyepiece for flexible endoscope. e. Digital Video Computer Module includes hardware and Windows-based software for fluoroscopy and FEES digital video recording, and hard copy color prints. Can be integrated with Swallowing Signals Lab and/or FEES components Computer module includes high-resolution (24-bit) broadcast quality video digitizer, and 19" XGA monitor. Systems allows concurrent acquisition and display of physiologic data acquired through Swallowing Signals Lab on same screen as video recording; quick, precise field-accurate correlation of video and physiologic data. High-fidelity powered speaker and lapel microphone for narration of recordings f. Color Printer, digital, which uses 8 " x 11" paper. Suitable for reports and Imagine printing. g. Swallowing Signals Lab Hardware and Software contains the signal acquisition Module, data acquisition card, and Windows-based software for real-time display, measurement, and real-time visual feedback of swallowing signals. The signals, when used in conjunction with Model 7145, can be displayed simultaneously with the video exam and analyzed with frame-specific accuracy post-acquisition. Possible inputs, with appropriate signal conditioning, include: Two channel surface EMG Six channel(or less) manometry (Gaeltec solid-state sensors) Multi-channel tongue array for lingual strength/symmetry assessment and therapy One channel for monitoring respiratory phase (using nasal cannula as input) One stethoscopic microphone input for acoustic analysis of swallowing or cervical auscultation Two auxiliary input channels (sampling up to 6kHz per channel) NOTE: requires isolation transformer to meet hospital safety regulations h. EMG Transducers (Two channels) contains 3 cables (1 provided as an extra) with snaps, 8mm disposable EMG sensors (quantity of 100), conductive gel and garment clip. i. Hardware Module & Tongue Array w/bulbs Each array consists of air-filled transducers that placed intro-orally for measurement of tongue strength and symmetry. Disposable, but reusable by same patient. Quantity of 25 arrays. j. Nasal Cannulas Standard nasal cannula used for viewing of inspiration, expiration, and apnea during swallowing. Disposable, but reusable by same patient. Quantity of 50. k. Laryngeal Microphone & Powered speaker Laryngeal stethoscopic microphone (1,500 Hz frequency response) for acoustic analysis of swallow. Includes powered speaker to enable patient and clinician to monitor swallowing and breathing aurally. l. Cart/Isolation Transformer custom designed cart to accommodate the complete Swallowing workstation. Isolation transformer included for hospital safety Regulations. m. Dual Endoscope Disinfection system: includes two endoscope holders mounted above two caddies. Caddies may be used for liquid disinfection agents. Systems is on side of cart. Salient Characteristics for the Digital Swallowing Workstation, Model 7200 are as follows: 1. Fees (Flexible endoscopic evaluation of swallowing.) 2. Digital Video Recording Model with Instant Playback and real time image 3. Swallowing signals labs to include surface EMG with tongue array sensors, nasal cannula for monitoring respiration and concurrent signals for multiple tests with viewing on split screen. CSL 4400 Clinical system Model 5293 OR EQUAL to include the minimum: a. Computerized Speech Lab (CSL) the CSL hardware/software system includes professional quality I/O system with: External module with input filter and amplifier, USB interface to host computer, Digital signal processor, Four- channel A/D & D/A (24-bit), 50kHz per channel), High fidelity output with studio speaker, AC & DC coupling, >90dB S/N, and 128:1 oversampling A/D. Includes SPDIF I/O for digital communication with DAT and other digital recorders/players. Includes latest version of the core CSL Software. Requires host computer with USB port win 95, 98, ME. b. Computer for CSL -- >550MHz Pentium III, 128 MB RAM, >15GB drive, 17" SVGA Monitor, CD-R/RW/ROM c. Condenser Microphone AKG acoustic's model C-410 Headset type microphone. Required for the Voice Range Profile. d. Auditory Feedback Tools this program provides a set of auditory feedback tools for speech training including looping, masking, pacing slow play and delayed auditory feedback (DAF). e. Disordered Voice Database: CD-ROM with 500 MB Database compiled at Massachusetts Eye and Ear Infirmary voice lab. Contains files and info on 650 patients. f. Games -- graphic which provide visual feedback for speech and voice exercises. g. Motor Speech Profile Analyzes and extracts parameters related to motor speech disorders. h. Multi-Dimensional Voice Program (MDVP) Calculates over 22 voice parameters and compares them to a normative database. i. Real-time Pitch provides real time display and analysis of pitch and energy traces. Also contains clinical protocols. j. Real-time Spectrogram This program generates real-time narrow and wide band spectrograms. k. Sona-Match Provides real time spectral biofeedback for vowel and sibilant training. l. Voice Range Profile (VRP) This program plots the patients voice amplitude range at all sustained fundamental frequencies. Salient Characteristics for the CSL 4400 Clinical system, Model 5293 are as follows: 1. Must be able to extract parameters related to voice and motor speech disorders. 2. Hardware as well as the software to take an analog signal and convert it to a digital signal in an environment with no external or internal noise to corrupt the signal, i.e. filters out the noise.
