Description

The Contractor shall provide all necessary personnel, supplies, transportation, property and malpractice insurance to provide outpatient, basic diagnostic, preventative and restorative dental services to head start children at Crownpoint, Fort Defiance, Gallup, and Shiprock Service Units. The Contractor shall provide basic diagnostic restorative and preventative dental services for the purpose of maintaining and improving oral health of Navajo head start children. Indian Health Service's policies and procedures shall be followed in the performance of this contract. Contractor shall provide dental services to an estimated 1580 head start children. Anticipated award is for a twelve-month period with two (2) one- year option periods. Interested offerors should submit their request for RFP M01-0020/PD, closing on 11 May 01, 4:00 p.m. local time.
