- This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6, as suppliemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written proposal is requested. THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-24. This solicitation is issued on a restricted basis as a total small business set-aside under NAICS Number 421450. The VA Medical Center, Asheville, North Carolina is activating a new ambulatory care addition. Construction is in progress, therefore it is essential potential offers be prepared to retain the equipment until the space is available for delivery. The anticipated date of completion for the space this equipment is going to be utilized in is August 15, 2001. The VA Medical Center reserves the right to extend the delivery date up to a maximum of 60 days at no additional cost. The requirement is for providing and installing three (3) each motorized x-ray viewers. Salient characteristics include: Vertical movement and viewing panels, controlled by an up-down footswitch. Two level illumination per divided 14"x17" viewing section is controlled by three way toggle switchs located on the switch console. A master switch for each tier is also supplied. The viewer shall have plexiglass panels and have top and bottom film grips. The viewer shall be able to hold a minimum of 300 each 14"x17" films (50 frams per unit) and display 12 each 14"x17" films simultaneously. The motorized viewer can reach any of the fifty panels in maximum time of one minute or approximately 2.5 seconds per panel. The viewer shall provide high illumination for darker films which can be controlled with a switch. Approximate size is 99.5" wide, 28.5" deep, and 93.5" height. Vendor shall provide color samples for selection of finish. Proposals for other than the Marconi Medical Systems MV 300A, Catalog number 242216 shall provide detailed specifications indicating electrical requirements. The VA Medical center shall provide electrical and site preparation prior to installation. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR ) Clause 52.212.1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeor's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification-Commercial Items. An authorized representative of the offeror must sign these reprsentations and certifications. The offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS Number) and Tax Identification number. The resulting firm fixed-price contract award will incorporate the requirements of the clauseat FAR 52.212-4, Contract Terms and Conditions-Commercial Items. Clause 52.212-5, Contract Termsnand Conditions required to Implement Statutes or Executive Orders-Commerical Items.
