Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. This solicitation no. 603-60-01 is a RFQ and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-25. The NAICS Code is 339112. This procurement is unrestricted. Clauses and Provisions from the FAR may be obtained online at http://www.arnet.gov/far and from the VAAR at www.va.gov/oa&mm/vaar. The VA Medical Center, Louisville, Kentucky has the requirement for one (1) each, Name Brand or Equal, Intravascular Ultrasound System,Model #S770470-INV, (Jomed/EndoSonics In-Vision Gold). System includes the following: Patient Interface Module (PIM); Magnet; Hanger; all systems cords; VCR tape; printer paper disk caddy; CD-R Microphone; Operators Manuals, and a one (1) year warranty. Equipment offered must be compatible and work in conjunction with existing Digital Mobile Neurovascular system. Offers should include descriptive litature for all equipment and options. Delivery shall be made to the VA Medical Center, 800 Zorn Avenue, Louisville, KY no later than 14 days after award. Delivery shall be FOB Destination. Provisions of 52.212-1, Instructions to Offerors, 52.212-2 Evaluation Commercial Items are applicable to this solicitation. The evaluation criteria included in paragraph (a): The following factors shall be used to evaluate offers: (1) Technical capability of the items(s)offered to meet the Govt's requirement which includes the compability to existing equipment; (2)Price, (3) warranty. These factors are listed in decending order of importance.
