- Description
- The Veterans Affairs Salt Lake City Health Care System (VASLCHCS) in Salt Lake City Utah requires the services of a contractor to provide delivery of medical equipment to beneficiaries of the VASLCHCS, 500 Foothill Boulevard, Salt Lake City Utah. Other requirements include picking up equipment from veterans homes, sanitizing, repairing and documenting preventative maintenance of equipment, instructing in usage of equipment, with written procedures as required. The NAICS code for this procurement is 621610. The size standard is $5 million. This procurement has not been set-aside for small businesses; it is offered for full and open competition. Offers will be evaluated on best value. Prior to best value evaluation, offerors must meet minimum requirements of the solicitation. Minimum requirements include: A) Submit evidence that your firm meets all state motor vehicle and driver licensing requirements, including insurance, for delivery vehicles and personnel proposed to work under this contract, including subcontractors. B) Submit evidence that your firm possesses liability and workers compensation insurance. C) Submit evidence that your firm is registered with the FDA and the DOT. D) Submit evidence that your firm has at least 36 months experience as a provider of durable home medical equipment. E) Submit evidence of ability to meet or exceed current Joint Commission for the Accreditation of Healthcare Organizations (JCAHO) standards. Such evidence might include JCAHO certificate of accreditation, equivalent accreditation, or QM/PI plan that addresses all applicable JCAHO standards. Whether JCAHO accredited or not, offeror must submit QM/PI plan addressing applicable JCAHO standards. JCAHO surveyors may visit contractor. Contract is for a period of one year with two one-year options. JCAHO accreditation must be applied for in the base year and will be required before exercising option for option year 1.
