Description

This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This procurement is being conducted under the Simplified Acquisition Procedures (SAP). NASA/GRC plans to acquire Dosimetry services for five years (one base year and four option years), to support the decommissioning activities of the Plum Brook Reactor Facility (PBRF) in Sandusky, Ohio. The services consist of the following: Work Package 1, "Whole Body and Extremity Monitoring; Work Package 2, "Whole Body Counting" Work Package 3, "Respirator Fit Testing" Work Package 4, "Radiobioassay" The services shall be performed on approximately 10 to 120 people during the life of this procurement. Offerors shall supply the following information in their quotes: (5 PAGE LIMITATION) -Quote for Base/Option Years for Minimum Level of required services -Quote for Base/Option Years based on price "per occurrence" -Quote for Base/Option Years for Options A and B under each Work Package -Subcontracting arrangements (if applicable) for any of the Work Packages(Company Name, Location, Service performed, Price) -List Past and Present experience in performing the services required -Copy of required Certification to perform the services (Prime/Subcontractor) The provisions and clauses in the RFQ are those in effect through FAC 97-25. All qualified responsible business sources may submit a quotation which shall be considered by the agency. Services to GRC Plumbrook are required on or about June 1, 2001 or as stated within the Statement of Work. Delivery shall be FOB Destination. Quotations for the items(s) described above are due by 4:00 pm EST, May 25, 2001 and may be mailed or faxed to NASA Glenn Research Center, Att: Irene Cierchacki(500-312), 21000 Brookpark Road, Cleveland, Ohio 44135, FAX (216)433-5489 and include, solicitation number, FOB destination to NASA Plum Brook Center, proposed schedule, discount/payment terms, CAGE Code number, taxpayer identification number (TIN), identification of any special commercial terms, and be signed by an authorized company representative.
