Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) is DLV10177 and incorporates provisions and clauses through FAC 97-25. This procurement is to be processed using Simplified Acquisition Procedures. The NAICS/Code is 339113/500EM. This is a 100% Small Business Set-Aside. The National Institute of Environmental Health Sciences is interested in obtaining dust/mist respirators. The dust/mist respirators should be Magic Arch N95 Particulate Respirator by Alpha ProTech Ltd or Equal. Estimated quantity for 1 year is 3330 boxes, 35 respirators to a box. The following is both the salient characteristics and criteria for evaluating "or equal" respirators: 1) Must be approved by NIOSH 42CFR84 to be N95 rated; 2) malleable aluminum band over the nose and under the chin to provide a custom fit for all users and create a positive facial lock; 3) Magic Arch design to provide maximum breathability with more functional breathing area, cooler comfort, sustained filtration and no restriction of downward vision; 4) eliminates fogging of safety goggles/glasses; 5) soft, hypoallergenic inner facial lining; and 6) two elastic retention straps to provide secure and easy application. If "equal" is being quoted, 1 box of respirators must be sent to NIEHS, Attn: Dee Lunney, 79 T.W. Alexander Drive, Bldg. 4401, Rm. 172, Research Triangle Park, NC 27709 by the closing date of this announcement. This is a Term Purchase Order (TPO) and items may be ordered on an as required basis by NIEHS during the period of 1 year. Delivery will be made to NIEHS Warehouse, Bldg. 104, South Campus, 111 T.W. Alexander Drive, Research Triangle Park, NC 27709. Offeror should quote FOB Destination. The following provision applies to this acquisition: 52.212-1, Instructions to Offerors. There are no additional addenda. Offerors shall include a completed copy of the provisions at 52.212-3, Offeror Representation and Certification-Commercial with offers. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial, applies to this acquisition. The following paragraphs of 52.212-5 apply: (a) and (11)-(16)(21)(22)(25)(27) of (b) and 52.211-6 Brand Name or Equal. Defense Priorities and Allocations Systems (DPAS) does not apply.
