Description

The Contractor shall provide Basic Life Support (BLS), Advanced Life Support (ALS) and non-emergency interfacility transport services for Fort Defiance Indian Hospital, Fort Defiance, Arizona. The Contractor shall provide all necessary personnel, pharmaceutical drugs, medical supplies, equipment, vehicles, facilities and other items and expertise necessary for or incidental to provide ambulance services. Services shall be available twenty-four hours per day, seven days per week. Anticipated award will be 01 Aug 2001 with two (2) one- year option periods. Interested offerors should submit their request for RFPM01-0022/PD, closing on 13 July 2001, 4:00 p.m. local time (Day Light Savings Time).
