Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number: RFP 504-01-015. This solicitation document and incorporated provisions are those in effect through FAC 97-27. Contractor shall supply equipment and materials OR Equal for the VA Medical Center, Amarillo, Texas. Equipment is as follows: Reliance, 7720NC, spackled finish floor unit, # 20 pearl; reliance 6200 chair (specify high or low); reliance 5356 pneumatic stool; Haag-Streit 900 BQ slit lamp; Haag-Streit BQ applanation tonometer; Heine omega 180 binocular indirect ophthalmoscope; Heine EN30 indirect wall transformer with extension cable (200cm); Reichert 11635B plus cylinder ultramatic rxmaster phoroptor refractor (black); Marco CP-670 automatic shart projector (head only); for Marco (wall mount)CP-600/670; Welch Allyn 11720 3.5v hallogen coaxial ophthalmoscope; Welch Allyn 18200 3.5v halogen streak retinoscope; Welch Allyn 71670 3.5v rechargeable handle; three well desk charger. Provision 52.212-1, Instructions to Offerors Commercial Item applies to this solicitation. Provision 52.212-2, Evaluation -- Commercial Items is not applicable. Offeror must include a copy of the provision 52.212-3, Offeror Representations and Certifications Commercial Items. Clause 52.212-4, Contract Terms and Conditions commercial Items applies, no addendum added. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies which includes the Service Contract Act.
