Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR part 13 and constitutes a formal Request for Proposal (RFP). A written solicitation will not be issued. The procurement is unrestricted and all responsible sources may submit an offer for consideration. Solicitation number is 502-43-01. The procurement documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-27 and VAAR # 21. The North American Industry Classification System Code is 621111 with $5.0 million annual sales limitation for small business. VA intends to award a firm-fixed price, non-personal services contract for Health Care Resources in accordance with Public Law 104-262 under the authority of Title 38 U.S.C. 8153. Requirement is One (1) Board Certified or Board Eligible Urologist one (1) day each week on Thursday from 8:00 AM to 4:30 PM at VAMC Alexandria LA. The Contract Urologist shall provide outpatient clinic services while the VA staff urologist performs surgery. Contract term is twelve (12) months with an option to extend services for an additional six (6) months. The contractor shall perform no service after September 30, until the Contracting Officer authorizes such services in writing. Responsibilities: Urologist is responsible for all facets of adult Genito-Urinary care as established by industry and VA standards. He/She shall be in good standing with the profession with knowledge and abilities current to equipment, medication and techniques. The Board Certified/Board Eligible Urologist shall provide comprehensive health services relating to all phases of Urological complaints and diseases, routine and malignant. The physician shall perform ambulatory surgical procedures to include, but not limited to cystoscopies, stent placement, urethral dilatations, and transrectal needle biopsies of prostate. There is no call back or overtime included. Clinic site is VAMC, 2495 Shreveport Highway, Building 7, Urology Clinic, Alexandria LA. Workload is approximately 30 patients per day. Services, including documentation, shall be performed in accordance with all VAMC established principles, ethics, standards, rules and regulations as set forth in VHA Directives, Medical Staff Bylaws, Medical Center Memorandums, AMA, and JCAHO standards. The Director of Information Management or his designee will address computer usage and security requirements. All data is proprietary and shall not be shared or manipulated in any manner. Contractor personnel shall observe the requirements imposed on sensitive data by law, federal regulations, DVA statutes and policy, and the associated requirements to ensure appropriate screening of all personnel. The Contracting Officer's Technical Representative, Harcharan Singh, M.D., Director of Specialty care Healthcare Line, will provide direction and monitoring of services. Any incidents of contractor non-compliance evidenced by the monitoring procedures shall be reported immediately by the COTR to the Contracting Officer. Teamwork and ability to work independently without supervision are essential. The Chief of Staff is the final authority for services provided under the terms of this procurement and any resulting contract and shall maintain an ongoing utilization review. All rules, regulations and laws applicable to privacy of patient, documentation and records apply. VA reserves the right to deny approval or to terminate the contract at any time for noncompliance. Payment may be made weekly or monthly in arrears upon receipt of a properly executed invoice. Invoice shall be submitted no later than the tenth of the month following the month in which services were provided. Contractor and VA will establish record keeping systems to verify contract performance. Preferred payment method is weekly in arrears using Government purchase card; however, other arrangements may be made. Services may be billed at an hourly rate or per procedure by CPT code. The primary applicable CPT codes are 99201, 99202, 99203, 99211, 99212, 99213, 55700, 5200, 52005, 52310, 52281, and 52332. The listed rate shall be an all-inclusive rate and no additional consideration will be made. The Contractor shall accept payment for services rendered under this contract as payment in full and shall not bill the veteran or a third party insurer for any services covered under this contract. No work is required on holidays. The ten holidays observed by the government include New year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving, Christmas and any other day proclaimed by the President of the United States to be a national holiday. Qualifications: The Contract Urologist shall have an active, current, unchallenged and unrestricted license in a state, territory, or commonwealth of the United States or District of Columbia and shall be board certified or board eligible with four or less years post residency. He/She shall speak, understand, read and write the English language fluently; be COMPUTER LITERATE, preferably with knowledge of the VA Computer Patient Record System(CPRS)/VISTA electronic medical records documentation access and input. Contractor personnel shall be trained in and given a copy of the current Automated Information System (AIS) Security Policy by the Service AdPac. This training is required prior to accessing any sensitive information and annually thereafter. Specific instruction for the VA electronic records systems will be provided; however, basic computer skills are essential. He/she shall be a US citizen or a permanent resident and possess required elements to be credentialed and privileged in accordance with VA regulations. Physician credentials/qualifications are subject to review and approval by the Medical Center Chief of Staff and the Medical Center Director. The Alexandra VAMC Credentialing and Privileging requirements must be met prior to acceptance at this facility. The Government shall not contract with any individual or business included in the OIG List of Excluded Individual Entities, Office of Inspector General List of Excluded Individuals, or vendors required to file a VETS-100 report but fail to do so. When applicable, the contractor shall be responsible for protecting the contracted staff assigned to this facility. The contractor shall provide worker's compensation, in-force malpractice professional liability insurance in an amount not less than $1,000,000 per occurrence for the entire contract period, health examinations, income tax withholding and social security payments. The Federal Tort Claims Act does not cover contractor personnel. When a contractor physician has been identified as a primary provider in a tort claim, the contractor physician is responsible for notifying the contractor's legal counsel and/or insurance carrier. Any settlement or judgment arising from contractor personnel action or non-action is the responsibility of the contractor and/or insurance carrier. Original Certificate of Insurance must be provided prior to acceptance of a physician. The Government provides no sick leave, vacation, or other benefits. The nature of this contract does not provide for any absence (i.e., sick leave, holiday or vacation time). Should an unavoidable delay occur, the contractor shall notify the COTR or designee at 318-473-0010, extension 2380 as soon as possible. At no time shall the contract physician be considered a VA employee for any purpose. The Contractor shall not supervise any VA employee and will perform only authorized contract work at the VAMC in strict accordance with the prices shown in the contract The Contracting Officer's Technical Representative shall monitor performance for contract compliance, knowledge, skills and behavior and immediately notify the Contracting Officer of any noncompliance or unacceptable behavior. The Department of Veterans Affairs reserves the right to deny approval or to dismiss from contract work, any physician having unsatisfactory performance in skills, knowledge, attitude, personality or for any other valid reason, or is considered by the Contracting Officer to be objectionable. Veterans shall be provided the same level of care the physician(s) provides to private patients. A contract may be awarded to one physician or a group of physicians. All physicians providing services at this medical center at any time require credentialing and privileging. The Government may award a contract based on documentation provided without further discussion. SUBMIT BACKUP DOCUMENTATION WITH OFFER. Offerors shall include the following with a proposal: pricing schedule, curriculum Vitae (CV), medical license , evidence of board certification/eligibility, Basic Life Support Certification and DEA certificate of proposed physician(s), three (3) references, and evidence of company's Indemnification and Medical Liability Insurance. The services specified in this contract may be changed by written modification to this contract, prepared by the VA Contracting Officer. Award will be made to the offeror whose proposal conforms to the requirements of the solicitation; best meets the minimum needs of the Government, and offers the lowest price. Best value to the Government and veteran shall determine award considering past performance, technical/ qualifications, and cost -- considering (1) Technical capabilities; (2) Past performance/experience and references; and (3) Cost. VAMC shall provide all support staff, nursing staff, clerical support, equipment and supplies for use in the performance of this contract.
