Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 and 15 and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. A written solicitation will be issued. Solicitation number 673-85-01 applies and is issued as a request for proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. The North American Industry Classification Systems (NAICS) code is 421460 at 100 employees. Provide eyeglasses and eye examination onsite and offsite services for eligible veteran beneficiaries of the Central Florida Veterans Health Care System, to include: James A Haley Veterans Administration Hospital (VAMC), Tampa, FL; Orlando Outpatient Clinic (OPC), Orlando, FL; Brevard (Viera) Outpatient Clinic (OPC), Viera, FL; Pasco Outpatient Clinic (OPC), Port Richey, FL; and Brooksville Community Based Outpatient Clinic (CBOC), Brooksville, FL. The Contractor shall fabricate all eyeglasses and provide comprehensive eye examinations for eligible veteran beneficiaries. Refractions and exams shall be performed by Optometrists or Ophthalmologists who possess a full and unrestricted license to practice in the State of Florida. Period of contract shall be for one from date of award. The Government may extend the term of this contract for two (2) optional years. A prebid meeting will be held at the James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL, date, time and location to indicated in solicitation. TECHNICAL QUESTIONS: Offerors should submit all technical questions regarding this solicitation to the Contracting Officer in writing and within 15 working days after issuance of this Request for Proposals (RFP). Questions received after the 15 days will not be considered. Verbal questions will not be addressed. All responses to questions, which may effect offers, will be incorporated into a written amendment to the solicitation. Subject to availability of funds. The above due date should be considered "on or about". Only written or faxed requests received directly from the requestor no later than are acceptable.
