- Description
- This is an emergency procurement using combine synopsis/solicitation for commercial services (Scarce Medical Specialist Resources of Locum Tenens), for two-locum tenen Psychiatrist for immediate placement through September 30. 2001. Prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR part 13. This announcement constitutes a formal Request for Proposal (RFP) and a written solicitation will not be issued. All responsible sources may submit an offer for consideration. Offers shall be considered only if clear and convincing evidence is provided documenting that the offeror can meet the requirements of the Government. Solicitation number is 502-49-01 and it documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 97-27 and VAAR #21. It is unrestricted and all responsible sources may submit an offer for consideration. The North American Industry Classification Code for Locum Tenens is 561330 and 621112 for physician offices, Mental Health Specialists for physician offices with $7.5 million annual sales for small business limitation. As a minimum, for each candidate under consideration, submit a curriculum vitae, a minimum of three references (names and phone number, previous employers and documentation sufficient to complete credentialing and privileging process education, board certification status, relevant training, experience, pending or settled challenges to license, and status of any claims. VA Application form 10-2850 must be completed in its entirety and is available upon request at 318-473-0010 ext. 2765 or 2202. Credentialing/Privileging prior to acceptance is essential. Requirement is two (2) Board Certified or Board Eligible Psychiatrist for approximately 3 months, commencing immediately. Services may be extended beyond the 90-day assignment by mutual agreement and availability of funds. (1) Estimated 500 hours @ $___________ $_________. This is an all-inclusive rate. No additional consideration will be provided for housing, travel, transportation, etc. No overtime, callback or holiday coverage included. Responsibilities: The Board Certified/Eligible Psychiatrists are responsible for comprehensive adult mental and behavior health services including assessment, diagnosing, management, & treatment of psychiatric problems with varying degrees of disturbances, provided at the Department of Veterans Affairs Medical Center, 2495 Shreveport Highway, Alexandria, LA. Duties include assisting patients to cope, set goals, communicate, maintain self-control, and improve self-reliance. Physicians are responsible for all facets, of psychiatric care and includes but is not limited to evaluation/assessment, development of treatment plans, management of substance and alcohol abuse, crisis intervention, counseling, individual therapy, proper documentation, and telephone consultations, Acceptable and proper documentation in accordance with all VAMC standards is essential and will be monitored by COTR. All services shall be documented and complete within 24 hours of occurrence. Computer expertise is essential to access and input medical documentation. Workload is approximately 12-15 patients per day. Services must be provided in accordance with all rules and regulations as set forth in VHA Directives, Medical Staff Bylaws, Medical Center Memorandums, and JACHO standards. All services shall be performed under the direction of the Director of Behavior Healthcare Line and the Chief of Staff. Teamwork is essential. All rules, regulations and laws applicable to privacy of patient, documentation and records apply. VA reserves the option to terminate sooner if VA recruitment efforts are successful or contract psychiatrist does not meet the needs of this medical facility and the veteran beneficiary in a professional, courteous and knowledgeable manner. Payment shall be weekly in arrears using Government purchase card for actual hours worked in a psychiatric care environment. Logs of work hours (certified sign-in/sign-out logs maintained by delegated VA staff) will be faxed weekly to the Contractor. Contractor shall fax corresponding invoices to 318-483-5063 for credit card information. Payment shall not exceed 40 hours per week. The hourly rate is an all inclusive rate and no additional consideration will be made. Qualification: Psychiatrist shall be licensed in a state, territory, or commonwealth of the United States or District of Columbia with current full, unchallenged and unrestricted license; shall be board certified or board eligible with less than four years post residency. The contract psychiatrist shall speak, understand, read and write the English language fluently; be computer literate with knowledge of Computer Patient Record System (CPRS)/VISTA electronic medical records documentation access and input. He/she shall be US citizen and must possess required elements to be credentialed and privileged in accordance with VA regulations. Physician credentials/qualifications are subject to review and approval of the Medical Center Chief of Staff and Medical Center Director. All documentation required to credential and privilege the physician must be provided to the Credentialing Office at this Medical Center prior to acceptance by this facility. OIG List of Excluded Individuals/Entities applies. Hours are Monday through Friday, excluding all federal holidays, 8:00 AM to 4:30 PM. No overtime, weekends, or callbacks authorized. Two physicians are required for the entire contract period. The ten holidays observed by the government include New Year's Day, Martin Luther King Day, President's Day Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran Day, Thanksgiving Day, Christmas and any other day proclaimed by the President of the United States to be a national holiday. Personnel Policy: The contractor shall be responsible for protecting contracted personnel assigned to this facility. The contractor shall provide worker's compensation, professional liability insurance in the amount of $1,000,000 per occurrence, health examinations, income tax withholding and social security payments. Sick leave, vacation, or other benefits are not the responsibility of the Government. At no time shall the contracted physician be considered a VA employee for any purpose. Documentation of current TB skin testing and record of Hepatitis B vaccination with current titer, or signed declination form, is required. Contractor Performance Monitoring: Monitoring hours worked will be accomplished with sign-in/sign-out sheets maintained by the secretary of BHHCL and verified by the Director of BHHCL documenting arrivals and departures. The Director of Behavior Health Healthcare Line will be the designated Contracting Officer's Technical Representative and shall monitor performance for compliance, knowledge, skills and behavior and immediately notify the Contracting Officer or any noncompliance or unacceptable behavior. Substitution or physicians is not allowed unless specifically authorized by the Contracting Officer, Director of BHHCL, and the Chief, of Staff. Any substitute must comply with all requirements and be credentialed and privileged. The Government may award solely from documentation provided without further discussion. SUBMIT BACKUP DOCUMENTATION WITH OFFER. Award will be made to the offeror whose quote conforms to the requirements of the solicitation; best meets the minimum needs of the Government and offers the lowest price. Best value to the Government and veterans shall determine award considering past performance, technical/qualifications and cost considering (1) Prior experience, demonstrated capability and continuity (two physician for the duration of the contract); (2) Past performance/experience and references (preferably VA); and (3) Cost.
