Description

Optometry services for the VA Medical Center, Louisville, KY. Contract period shall be for one (1) year from date of award with the option to renew for an additional one-year period. Estimated requirements are for approximately 5,200 exams per year. Contractor will provide the facility(s), staffing, equipment and supplies required to perform the services. All beneficiaries referred will be seen at the Contractor's place(s) of business. VA intends to award a firm fixed price contract(s) to provider(s)located within the VAMC, Louisville, KY catchment area. Requests for the solicitation package must be in writing and must reference the solicitation number and description. NAICS code 621320 applies. Award is subject to availability of FY funding. The solicitation package will be available on or about August 10, 2001.
