Description

This procurement is hereby issued for commercial items to be used by the VA and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12, Acquisition of Commercial Items, Subpart 12.6 and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. Solicitations are being requested under Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program. The NAICS code is 62112 and the solicitation is 100% Set Aside for Small Business. Provide physician Board Certified or Board Eligible in gynecology to conduct one clinic per month at the VA Medical Center, Fayetteville, Arkansas in accordance with the terms and conditions of the contract. Contract period is for one year with four one-year options to extend the term of the contract. The NAICS code is 62112 and the solicitation is 100% Set Aside for Small Business. TECHNICAL QUESTIONS: Offerors should submit all technical questions regarding this solicitation to the Contracting Officer in writing, within (10) calendar days after issuance of this Request for Quotation (RFQ). Questions may be sent via facsimile at (501) 444-5031.Questions received after the 10 days will not be considered. Verbal questions will not be addressed. All responses to questions, which may affect offers, will be incorporated into a written amendment to the solicitation.
