Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number R6-15-01-90 is issued as a Request for Quotations (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27. (iv) This procurement is unrestricted; The standard industrial code is 7629 (NAICS code 811219) and the small business size standard is $5,000,000. (v) Quoters must proved price quotations as follows: item 1a base year (Oct. 1, 2001 Sep. 30 2002), maintenance and repair of UPS and battery pac, quantity -- 12, unit -- month; item 1b option period 1 (Oct 1, 2002 -- Sep 30, 2003), maintenance and repair of UPS and battery pac, quantity -- 12, unit -- month; item 1c option period 2 (Oct. 1, 2003 -- Sep. 30, 2004), maintenance and repair of UPS and battery pac, quantity -- 12, unit -- month. (vi) The agency need is for a service/maintenance contract for one (1) each government owned EPS-2030/22,66 uninterruptible power system (UPS) and BPA 2000/342BJD battery pac. The statement of work is as follows: 1. The contractor shall furnish all materials, equipment, labor, and travel necessary to calibrate, adjust, test, do monthly preventive maintenance and repair the government equipment listed herein. This work does not include battery replacement. 2. The contractor agrees that all service shall be preformed on-site by trained and certified technicians qualified to work on the equipment listed herein. No equipment or any part of the equipment shall be removed from site without written authorization from the COR. 3. The contractor shall perform preventive maintenance on a monthly basis to prevent equipment breakdown and to ensure availability of a functional system to effectively serve its intended purpose. The contractor shall furnish and install all necessary repair parts and perform calibration, adjustment, safety and operational checks necessary to ensure the equipment meets manufacturer's specifications. Replacement parts used must be new and/or certified to comply with the manufacturer's specifications. 4. There shall be unlimited emergency services during normal working hours, 8:00 AM to 5:00 PM, Monday through Friday. (Saturdays, Sundays, and Holidays are excluded.) Response time shall be within 24 hours from the receipt of the emergency service request. Emergency service which is not attributable to maintenance, is not provided for under the terms of this contract. A separate purchase order will be negotiated for these repairs. 5. The contractor shall complete and provide a service report which shall include but not limited to the following information: a) Date and name of contractor's representative performing the service b) Labor hours expended c) List of parts used and cost d) Brief summary of the services and corrective actions taken. (vii) The equipment is located in the Forest Supervisor's Office, Roseburg, Oregon. The contract will be for a base year and two option years commencing on 10/1/01. This requirement is for a fixed priced contract. (viii) The provisions at FAR Clause 52.212-1, Instructions to Offerors -- Commercial Items (OCT 2000) and the following clauses incorporated by addendum apply -- At paragraph (c), "Period for Acceptance of Offers" is amended to read 60 calendar days; (e), "Multiple Offers," has been determined by the Contracting Officer to be non-applicable to this solicitation and is hereby removed in its entirety for the purpose of this requirement; (h), "Multiple Awards," has been determined by the contracting Officer to be non-applicable to this solicitation and is hereby removed in its entirety for the purposes of this requirement; 52.215-5 Facsimile Proposals (OCT 1997) -- Fax Number is (541) 957-3495. (ix) FAR Clause 52.212-2, Evaluation -- Commercial Items (JAN 1999) apply -- At paragraph (a) The following factors shall be used to evaluate offers: price, technical capability, past performance, and business size standard. (x) FAR Clause 52.212-3, Offeror Representations and Certifications -- Commercial Items (MAY 2001) is incorporated and are required to be submitted with the contractor's quote.
