Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotes are being requested and A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number RFQ N00244-01-T-1108 applies. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-27 and DFAR Change Notice 20001213. The North American Industry Classification System code is 811219 and the business size standard is $5.0 mil. The agency need is for the following: Provide maintenance service for the period of one year commencing 01 Octobrer, 2001, for GE Radiology Systems located at the Naval Hospital, Lemoore, CA. Systems to be covered under the mai ntenance contract are: 0001 XRAY Machine, Model-46-27600462, Serial #293K56WK6, System #209998RF1 Location-Room 1 0002 XRAY Machine, Model-46-2742061, Serial #446825WK2, System #209998RF2 Location-Room 2 0003 XRAY CT Machine, Model-HISPEED DX/I, System#559-998LCT 0004 MAMO XRAY Machine, Model-Z108143, Serial #155909BU5, System #209998LNSE02 Location Room 1234 0005 C-ARM Stenoscop D9 Mobile, System #209998CARM, Location Operating Room Full service to include all parts, labor, travel, unlimited emergency call back 24 hours a day, seven days a week. Minimum response time is sic hours for items 0001 through 0004. Response time for item 0005 is immediate. The contractor will report to the BIO-Medical Division, room 1115, Bldg 937, prior to commencing service and upon completion of service. The contractor shall submit a field service report to the BIO-Medical Division with the following information: a. The nature of complaint b. Identification of equipment serviced c. Model and service number d. Activity bar code e. Type of service f. Itemized list of parts and supplies including costs for individual items g. Total manhours to include travel time .......................................................................................................................................... The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items applies. Addendum to FAR 52.212-1, Paragraph (b)(5): Offers shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties, offered to the general public. Express warranties shall be included in the contract. Clause 52.212-4, Contract Terms and Conditions -- Commercial Items applies as well as the following addendum clauses: FAR 52.219-1, Small Business Program Representations; FAR 52.211-5, Material Requirements. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders -- Commercial Items applies with the following applicable clauses for paragraph (b): FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items, applies with the following clauses applicable for paragraph (b): DFARS 252.225-7001, Buy American Act and Balance of Payment Program; and DFARS 252.225-7012, Preference for Certain Domestic Commodities; and addendum provisions DFARS for Certain Domestic Commodities; and addendum provision DFARS 252.211-7003, Brand Name or Equal. OFFERORS ARE REQUIRED TO COMPLETE AND INCLUDE A COPY OF THE FOLLOWING PROVISIONS WITH THEIR PROPORALS: FAR 52.212-3, Offeror Representation and Certifications -- Commercial Items. The government intends to make a single award to the responsible offeror whose offer is the most advantageous to the government considering price and price-related factors. Provision 52.212-2, Evaluation -- Commercial Items, applies with paragraph (a) completed as follows: Award will be made to the offeror that meets the solicitation's minimum criteria for technical acceptability at the lowest price. To be determined technically acceptable, the offeror must furnish product literature that demonstrates the products offered meet all requirements stated in the above purchase descriptions. EVALUATION CRITERIA: Award will be made on a best value basis taking into account the following factors: te chnical acceptability, past performance, and price. The Government may award to other than the lowest priced offer based on a trade-off among these factors.
