Description

This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation. Written quotations will not be issued and each contractor is expected to respond on his/her letterhead with all required product information. RFQ 621-01-0151 is hereby assigned to this procurement and shall be referenced on all correspondence relating to this acquisition. This RFQ is issued using Simplified Acquisition Procedures and incorporates all provisions and clauses as those in effect through FAC 97-27 and VAAR 97-1. The James H. Quillen VAMC, Mountain Home, TN 37684 has a requirement to purchase one (1) Garson-Stadler, Inc., Series 1200 Audiology Test Booth with an outside dimension of 10' x 10', booth door will be on the left (hinged left) and window on the right, BRAND NAME OR EQUAL and Installation of the Audiology Test Booth. Pricing to include shipping costs. The delivery date will be no later than 90 days after award of a contract. Contract award is subject to the availability of funds. The following provisions apply to this acquisition: 52.212-1, Instructions to Offerors Commercial Items; and 52.212-3, Offeror Representations and Certifications Commercial Items.
