Description
Firm fixed-price Architect-Engineering (A-E) services are required for the preparation of construction documents to include plans, specifications, and a cost estimate for bidding purposes for Ambulatory Surgery Consolidation, Building 57, SAVAHCS. Construction period services include preparing record drawings; and reviewing shop drawings, requests for information (RFI's) and submittals. The contract will be of the Firm Fixed Price type. This contract will be a design services contract and shall include architectural, mechanical, fire protection, electrical and structural disciplines. Medical planning and medical equipment planning is also required. The requirements for this project are as follows: Proposed project will create a consolidated ambulatory surgery unit on the 2nd floor of Building 57 directly adjacent to the existing OR's. Presently, the ambulatory surgery unit space is inadequate to meet the needs of this growing program. The ambulatory surgery unit was implemented in 1991 and has grown in size and scope since inception. The space limitation of the current ambulatory surgery unit limits the availability of recovery in this area to Secondary Recovery only. Primary recovery is performed in the Post Anesthesia Care Unit (PACU). Post surgical care is thus fragmented into three separate locations: (1) inpatients in the post anesthesia care unit (PACU); (2) same day surgery receive their primary recovery in the PACU and then are transferred to the Ambulatory Surgery Unit for secondary recovery; and (3) other invasive procedures receive their secondary recovery on the 3rd floor of Building 2. This project will consolidate these activities directly adjacent to the OR's where the current PACU is located, while creating a new intake and waiting area directly adjacent to an elevator. The consolidated unit will allow for more efficient staffing as well as improve patient care and satisfaction to all programs. The A-E must demonstrate his and each consultant's qualifications with respect to the published evaluation factors for design and all option services. Evaluation factors (1) through (6) are of equal importance; factor (7) is of lesser importance than factors (1) through (6). Specific evaluation factors include: (1) Professional qualifications of the firm and staff including those of any consultant, proposed for designing the project described above; (2) Specialized experience and technical competence in the type of work described above. Firm's prior experience should include, at a minimum, completion of projects associated with health care facilities, to include emergency power installation and code compliance issues where appropriate; (3) Capacity of the firm and project teams to accomplish subject project; (4) Past performance on contracts with Government agencies (emphasis on VA work) and private industry in terms of cost control, quality of work, and compliance with performance schedules; (5) Specific internal quality control procedures. Firm will be evaluated on the acceptability of their internal quality control program used to ensure technical accuracy and discipline coordination of plans and specifications; (6) Location in the general geographical area of the anticipated projects (within 125 miles of Tucson) and knowledge of local site conditions; and ability of the firm to provide timely response to requests for on-site support. (7) The volume of work previously awarded to the firm by the Department of Veterans Affairs. Firms which meet the requirements listed in this announcement are invited to submit one copy each of the completed Standard Forms (SF) 254 and SF 255 including each sub-consultant's current SF 254 to Mark Mikus, Code 9-90C, SAVAHCS, 3601 S. Sixth Ave., Tucson, AZ 85723 not later than 2:00 P.M. (PST) October 2, 2001. Fax copies of the SF 254 and 255 will not be accepted. Late responses will be handled in accordance with FAR 52.215-10. No material will be returned. THIS IS NOT A REQUEST FOR PROPOSAL and NO solicitation package or bidder/planholder list will be issued.
