Description

Subject Solicitation is for commercial items and is being processed in accordance with the format in Subpart 12.6. It is being issued as a Request for Quote (RFQ) using Simplified Acquisition Procedures (SAP) authorized in Subpart 13.5 Testing Program for Certain Commercial Items. Madigan Army Medical Center, Tacoma WA intends to establish a firm, fixed-price purchase order for Misc. Medical Equipment and Supplies. The RFQ Number is DADA13-01-T-0098, the NAICS code is 421450 with a size standard of 500 employees. This requirement is being processed as full and open competition; however, it may be converted to a small business set-aside without further notice if quotes are obtained from two or more small businesses who are able to provide acceptable products that are priced fair and reasonable from small business manufacturers on a line by line basis. Requests for copies of the RFQ shall be submitted in writing and submitted electronically via email to kathleen.benjamin@nw.amedd.army.mil or facsimile at 253-968-4922. Copies of the RFQ will be available on or about 04 September 2001 with an anticipated closing date of 14 September 2001. The request should include business name, point of contact name, complete address, telephone number, solicitation number, business size, tax identification number, cage code and Dun and Bradstreet Number. Telephonic requests will not be honored. All responsible sources may submit an offer, which will be considered. Firms offering a response to this notice should ensure registration in Central Contractor Database prior to submission for consideration.
