Description

Full service agreement for preventive maintenance and repair of the Cardiac Cath Lab equipment. System includes: Philips OM200 General XRE Positioner Philips Table Philips Overhead Crane Camtronics Digital Acquisition Imager Arritechno Camera Cine Two PM's are required per year. The first due 10/15/01 and the second due on 4/15/02. Service hours are Monday Friday 8:00am to 5:00pm. Any unscheduled work shall be at the prevailing rate. Service engineers maintaining this system must be certified and have a letter stating this on file in the Biomed section of the VA. Exclusions: All glassware, x-ray tubes, image intensifiers, CRT's and camera tubes are exempt as well as damaged parts due to neglect. Solicitation available on or about September 12, 2001. The contract will be for a one year period with two (2) one year options. All requests must be in writing and contain you full street address, telephone, and fax numbers.
