Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number is F31601-01-T-0162 and the solicitation is issues as a Request for Quotation (RFQ). (iii) This solicitation document incorporates provisions and clause in effect through Federal Acquisition Circular 97-27. (iv) This solicitation is 100% set-aside for small business concerns. The associated NAICS code is 621339 and small business size standard is $5,000,000.00. (v) This requirement consists of one line item to be priced on an hourly basis for twelve months. This requirement also contains four option years, to also be priced on an hourly basis for 12 months each respectively. (vi) The requirement is for the Dietitian Position at the Pope AFB's Health and Wellness Center. The position is needed Monday through Friday with 1 hour lunch. 2080 hours -80 hours for Federal holidays, -240 hours for vacation and sick time, -80 hour for continuing education showing a total of 1680 hours billable for contract. A registered Dietitian with a minimum of 5 years experience, Masters Degree in a related field is needed to provide medical nutrition therapy in an outpatient setting. Must have experience using Composite Health Care System and Ambulatory Data System. Must be licensed by the state of North Carolina and must be current with the Commission on Dietetic Registration. Background check is required. The expected period of performance of any resulting contract will be 1 Oct 01 through 30 Sep 02. The four option years performance shall be 1 Oct 02 through 30 Sep 03, 1 Oct 03 through 30 Sep 04, 1 Oct 04 through 30 Sep 05, 1 Oct 05 through 30 Sep 06, respectively. Government office space will be provided during the performance of any resulting contract. (viii) The following clauses/provisions are applicable: FAR 52.212-1, Instructions to Offerors-Commercial Items. (ix) FAR Clause 52.212-2, Evaluation-Commercial Items, paragraph (a) is added to read: The following are the significant evaluation factors ranked in descending order of importance: 1) Registered Dietitian with 5 years experience. 2) Master Degree in related field. 3) Licensed by State of North Carolina. 4) Current with Commission on Dietetic Registration. 5) Experience with Composite Health Care System and Ambulatory Data System. The contractor certifies that it meets all requirements by submitting a quotation. The contractor may be asked for references to verify that the workers met all requirement of the solicitation. This solicitation is issued under lowest price technically acceptable procedures. (x) FAR Clause 52.212-3, Offeror Representations and Certifications-Commercial Items. The contractor shall return a completed copy of this provision with its quotation. A copy of the provisions may be obtained from http://www.arnet.gov/far. (xi) FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items paragraph (f) is added to read: An individual worker's sick day is not considered an excusable day. Paragraph (i) is added to read: Payment will be made on a monthly basis. (xii) FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, paragraphs (b)(12),(13),(14),(15),(16), and (24). Paragraphs (c) (1),(2), and (3). The Department of Labor Wage Determination Number 1994-2393, Revision Number 21 applies to this acquisition. FAR Clause 52.217-7, Option for Increased Quantity-Separately Priced Line Item, is added to read 30 calendar days. FAR Clause 52.217-9, Option to Extend the Term of the Contract, is added to read 15 calendar days and 60 calendar days, respectively. Paragraph (c) is added to read: 60 months. (xiii) Not used. (xiv) Defense Priorities and Allocations System (DFAS) is not applicable to this acquisition. (xv) Commerce Business Daily numbered Note 1 applies to this acquisition. (xvi) Contractors must be registered in the Central Contractor Registration (CCR) system to be eligible to receive this award. All quotations with Resumes are due not later than closed of business, 4:15 pm eastern standard time, on 14 Sep 01. Quotes can be faxed to (910) 394-6279.
