- Description
- Provide all labor, materials, and supervision necessary to accomplish Home Oxygen Services for RFP 668-309-01 in accordance with specifications. This is an unrestricted solicitation with Hubzone accomidations for small businesses. NAICS Code 532291 Size Standard 5.0 Million. Requests must be made in writing for the specification package. Requests for bids will only be accepted from responsible businesses currently engaged in this type of commerce. Estimated project value is $500,000 to $1.0 million over the course of the 1-year contract with four(4) one(1) year options to renew. A Firm Fixed Price contract is anticipated. Project documents are available immediatedly (September 4, 2001). Contractor to provide COMPLETE HOME MEDICAL OXYGEN SERVICES at beneficiaries residence for the Department of Veterans Affairs Medical Center, 4815 N. Assembly Street, Spokane, WA 99205, in accordance with the terms and conditions herein, for the base period of October 1, 2001, through September 30, 2002, with four (4) one year options to renew. If all options are exercised the final expiration date will be September 30, 2006. SERVICE AREA (STATE/COUNTIES): a. The contractor agrees to provide service to all beneficiaries authorized by the Department of Veterans Affairs Medical Center, Spokane, WA that reside in the counties awarded in this contract. b. Servicing Area (State, Counties): This contract provides for the required services to be performed within the beneficiaries' residence located in: 1. Eastern Washington (Okanogan, Douglas, Grant, Ferry, Lincoln, Adams, Stevens, Spokane, and Whitman counties) 2. Northern Idaho (Boundary, Bonner, Kootenai, Benewah, Latah and Shoshone counties) 3. Western Montana (Lincoln and Sanders counties) Service will include: -- Initial delivery and set-up of equipment -- Pickup and storage of equipment -- Servicing of government-owned equipment per manufacturers specifications -- Supply of consumable parts and supplies -- Rental of equipment -- Refilling oxygen cylinders/reservoir -- Instruction to beneficiary or family member on the care and use of equipment
