Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation. Request for Quotations (RFQ) are being requested and a written solicitation will not be issued. The request for quotation number is F08602-01-T-5045. This solicitation incorporates provisions, which are in effect through Federal Acquisition Circular 97-15. This procurement is 100% set-aside for small business concerns. The Standard Industrial Code (SIC) is 8999. The small business size standard is $5,000,000. The performance period is 01 October 2001 -- 30 Sep 2002. A Purchase Order will be issued. Interested offerors shall provide as a response to this quotation: (1) a price based on the described requirements, and (2) a completed clause 52.212-3. Quotes may be submitted hard copy or electronically, via e-mail at charlene.bostic@macdill.af.mil or fax at 813-828-4508. Regardless of method of delivery, it is the offeror's responsibility to ensure receipt of the quote by the time and date stated on this combined synopsis/solicitation. The following is a description of the services required: MacDill Air Force Base, Florida has a requirement to procure preventative maintenance service for the G.E Prestelix1600 x-ray room#3, G.E. Advantx x-ray room #4, and G.E. MVP 80 x-ray room #5. The provision at FAR 52.212-1, "Instructions to Offerors-Commercial Items" is applicable to the solicitation and the blanks in (a) are completed as follows: price. Offerors shall include with their offers a completed copy of the provision at FAR 52.212-3, "Offeror Representations and Certifications-Commercial Items", FAR 52.212-4, Contract Terms and Conditions-Commercial Items is applicable to this solicitation. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items is applicable and the following clauses are checked: 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration. FAR 52.232-18, Availability of Funds, and 52.219-6, Total Small Business Set-Aside are applicable. DAFRS 252.204-7004, Required Central Contractor Registration (CCR) is applicable to this solicitation.
