This is a combined synopsis/solicitation for commercial item prepared in accordance with the Federal AcThis is a combined synopsis/solicitation for commercial item prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written proposal is requested. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. This solicitation is issued on an unrestricted basis. The Central Arkansas Healthcare System, Little Rock, Arkansas has a requirement for service on station to replace dialysis water system consisting of (1) Reverse Osmosis Unit, (1) Softener, (6) Service Carbon, (1) Multimedia Filter, (2) Service Deionizers, (1) Final Filter, (1) Quality Control Indicators. The contractor must provide all installation materials and labor. The system must meet the demand of 23 stations, (6 gpm), and bicarb fill, (1.5 gpm). The system must comply with AAMI and FDA requirements and have received 510(k) clearance. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Comercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors, therefore the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. OFFERORS SHALL INCLUDE A COMPLETED COPY OF FAR CLAUSE 52.212-3, OFFEROR REPRESENTATION AND CERTIFICATIONS. An offeror shall also include its Commercial and Government Entity (CAGE) code Contractor Establishment Code (DUMS Number) and Tax Identification number. The resulting firm fixed-price contract award will incorporate the requirements of the clauses at FAR 52.212-4, Contract Terms-Commercial Items, FAR 52.21205, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial Items.
