- The Department of Veterans Affairs, John D. Dingell Medical Center is seeking request for quotes to provide standard maintenance (parts and labor) on 17 anesthesia ventilator units. Manufacturer is Ohmeda. One each of model # Excel 210, eight each of model # 7800, and eight each of model # Mod II SE. This is a combined synopsis/solicitation for a commercial item, prepared in accordance with the subparts in Federal Acquisition Regulation 12.6 as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. Provisions and clauses found at 52.212-1, 52.212-4, and 52.212-5. The North American Industry Classification Code is 811219, Medical and surgical eqipment repair and maintenance services. The very small business size standard is 15 or less employees. Offerors may find the FAR provisions on the internet at www.arnet.gov/far. Proposed rates should include all costs associated with providing the following service. The accurate performance of these duties not only has an impact on the quality of patient care, but also the effectiveness and efficiency with which care can be provided. All responses must include a list of replacement items that are normally used when performing standard maintenance. Contractor will be required to perform preventive maintenance within ten working days of notification from Biomedical Engineering. Contractor will be required to attend Safety Training as required by the Safety Office of the VA Medical Center. This announcement constitutues the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation Number Request (RFQ) 553 -- 85-01 is assigned and shall be referenced on any proposals. Partial proposals shall be rejected. All potential offerors shall be available to perform the requirements of this contract immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Comnmercial Items. Offerors must also submit with their bids written evidence of training and proficiency (state of competency) in repairing or maintaning the equipment on which they are working. Contract term will be for one base year and four option years. Proposals shall be submitted to the address listed herein or faxed to 313-576-1077 and directed to the attention of Marilyn Dogan (001R-A). The government shall not be held responsible for quality of fax transmittals. The solicitation number (RFQ number) shall be listed on the outside of your response for identification.
